Resumen
Introducción y objetivos

La puesta en marcha de los servicios extendidos de medicina intensiva (SEMI) puede influir en el momento del ingreso de los pacientes en el servicio de medicina intensiva (SMI).

Material y métodos

Estudio de cohorte retrospectivo, descriptivo, cuasi-experimental «before-after». Se incluyeron todos los pacientes que ingresaron en la unidad de lunes a viernes de manera urgente de 2 periodos (entre el 1 de febrero de 2022 y el 30 de junio y entre el 1 de febrero de 2023 y el 30 de junio de 2023). Se dividieron a los pacientes entre ingresados en turno ordinario (08-15h) y de guardias (15-08h) Como objetivo secundario fue valorar si existían diferencias en la mortalidad entre ambos periodos.

Resultados

Durante el primer periodo del estudio ingresaron 239 pacientes. El 29,29% ingresaron en el turno ordinario y el 70,71% en turno de guardia. Durante el segundo periodo se incluyeron 211 pacientes con un 43,13% de ingresos en turno ordinario. La comparativa entre los 2 periodos objetivó un aumento del porcentaje de ingresos en horario de mañana en el segundo periodo de manera significativa (p=0,0031). La mortalidad en el primer periodo fue del 13,80% y en el segundo periodo del 9,95%. La comparación entre los dos periodos no objetivó diferencias significativas.

Conclusiones

La puesta en marcha de SEMI se asocia a que aumente la proporción de ingresos en la unidad de medicina intensiva en el periodo de mañana sin haberse objetivado cambios en la mortalidad.

Abstract
Introduction

The implementation of the Critical Care Outreach Teams can influence the time of admission of patients to the Intensive Care Unit (ICU).

Material and methods

Retrospective, descriptive, quasi-experimental “before-after” cohort study. All patients admitted to the unit urgently from Monday to Friday for two periods (between February 1, 2022 and June 30 and between February 1, 2023 and June 30, 2023) are included. The patients were divided into regular shift admissions (08-15h) and on-call (15-08h). The secondary objective was to assess whether there were differences in mortality between the two periods.

Results

During the first period of the study, 239 patients were admitted. 29.29% entered the ordinary shift and 70.71% on duty shift. During the second period, 211 patients were included with 43.13% of admissions in the ordinary shift. The comparison between the two periods observed a significant increase in the percentage of admissions in the morning hours in the second period (P=.0031). Mortality in the first period was 13.80% and in the second period 9.95%. The comparison between the two periods did not reveal significant differences.

Conclusions

The start-up of the Critical Care Outreach Teams is associated with an increase in the proportion of ICU admissions in the morning period without any observed changes in mortality.
