La disfunción tiroidea durante la gestación repercute sobre la salud maternofetal y puede influir en el desarrollo neurocognitivo del niño. La fisiología tiroidea cambia durante el embarazo y obliga a establecer intervalos de referencia específicos por trimestre y para cada población y método. Los objetivos de nuestro estudio fueron analizar la función tiroidea a lo largo del embarazo y establecer los intervalos de referencia de TSH y T4L en cada trimestre para nuestra población y metodología.
Material y métodosEstudio analítico prospectivo de 598 gestantes que abarca desde marzo de 2018 hasta octubre de 2020. En todas ellas se determinaron TSH, T4L, T3L, ATPO y ATG. Se excluyeron 151 gestantes por presentar inmunidad tiroidea positiva, enfermedad tiroidea previa en tratamiento con levotiroxina, embarazo gemelar, diagnóstico de hipotiroidismo e hipertiroidismo en la petición o ausencia de algunos de los parámetros estudiados, siendo la población de referencia de 447 gestantes.
ResultadosLos valores de referencia para la TSH fueron 0,07-3,14mUI/l para el primero, 0,66-3,21mUI/l para el segundo y 0,52-2,97mUI/l para el tercer trimestre. Los valores de referencia para la T4L fueron 0,81-1,19ng/dl para el primero, 0,71-1,07ng/dl para el segundo y 0,69-1,06ng/dl para el tercer trimestre.
ConclusionesLos intervalos de referencia para TSH y T4L obtenidos en este trabajo difieren de los utilizados para la población general, por lo que se han podido generar errores en la clasificación y tratamientos innecesarios en las gestantes.
Thyroid dysfunction during gestation impacts on maternal-fetal health and may influence the neurocognitive development of the child. The objectives of our study were to analyse thyroid function throughout pregnancy and to establish reference levels for TSH and T4L in each trimester for our population and methodology.
Material and methodsProspective analytical study of 598 pregnant women from March 2018 to October 2020. TSH, T4L, T3L, ATPO and ATG were determined in all of them. A total of 151 pregnant women were excluded due to positive thyroid immunity, previous thyroid disease in treatment with levothyroxine, twin pregnancy, diagnosis of hypothyroidism and hyperthyroidism in the request or absence of some of the parameters studied, with a reference population of 447 pregnant women.

ConclusionsThe reference levels for TSH and T4L obtained in this study differ from those used for the general population, which may have led to misclassification errors and unnecessary treatment in pregnant women.
