El paciente lesionado medular puede presentar disfonía severa por parálisis laríngea recurrencial secundaria a la cirugía aplicada para el tratamiento de su lesión medular. Su incidencia es baja, un 2,4% del total de pacientes lesionados medulares intervenidos quirúrgicamente en nuestro hospital. Se presenta el estudio realizado a siete pacientes (cinco pacientes presentaban tetraplejia y dos paraplejia) con disfonías severas por cirugía en el que se les realizó: a) exploración laríngea por laringoscopia indirecta antes y después de finalizar el tratamiento rehabilitador como medida diagnóstica; b) análisis acústico de voz para evaluar la gravedad de la disfonía al inicio, durante la terapia y finalizada la misma (software Dr. Speech Sciences); c) rehabilitación logopédica específica para este tipo de pacientes.
A los pacientes se les diagnosticó parálisis laríngea recurrencial unilateral con disfonía severa en el 100% y disfagia moderada para líquidos en el 71%; finalizada la terapia dos pacientes presentaban movilidad normal de larínge y el resto mantenía paralizada la cuerda vocal en posición mediana realizándose una correcta función glótica, siendo la voz y la deglución normales, y el parámetro acústico que mejor reflejó el grado de recuperación fonatoria fue el ruido glótico (NNE), con una media de 11, 48 dB.
The patient with spinal cord injury can present severe vocal disorder because of recurrent larynx paralysis secondary to the surgery applied for the treatment of his spinal cord injury. The frequency of the total patients with this problem who had surgery in our hospital is low (2,4%). We present the study to 7 patients (5 patients presented tetraplegic and 2 paraplegic) with severe disphonia after surgery in which they were made: a) A larynx exploration by indirect laryngoscope before and after finished the rehabilitating treatment as a diagnostic measure; b) An acoustic analysis of the voice to evaluate the severity of dysphonia at the beginning, during the therapy and when finished (software Dr. Speech); c) A logopedic specific rehabilitation for this kind of patients.
All the patients were diagnosed of unilateral recurrent larynx paralysis with severe disphonia in the 100% and moderate dysphagia for liquids in the 71%. Ended the therapy 2 patients presented a normal larynx movement and the rest maintained paralyzed the vocal cords in a medium position making a correct glottis function, being normal the voice and the swallowing. The acoustic parameter that best showed the grade of phonetic recovery was the Glottis Noise (NNE) with an average of 11, 48 dB.
