Abstract
Introduction

Catamenial pneumothorax (CN) is a rare type of pneumothorax that affects women of childbearing age. Its etiopathogenesis is controversial and several theories have been proposed, although there is a frequent relationship with intrathoracic endometriosis. The diagnosis can be a challenge, so recurrences can appear until it is found and it can be treated correctly. This document describes a case of catamenial pneumothorax and develops a care plan tailored at a patient suffering from this type of pathology.

Case description

A 41-year-old woman, with no personal or family history who attended the emergency department reporting pain in the right hemithorax and dyspnea. The chest X-ray revealed a complete pneumothorax that required placement of a chest drain. During the surgical intervention, cicatricial lesions with gray and black diaphragmatic pores are observed, which suggests a pneumothorax of catamenial origin.

Assessment

The nursing assessment was designed using the conceptual model of Marjory Gordon, where the patterns of activity-exercise, rest, perception and sexuality-related patterns were identified as altered.

Diagnosis

Using the taxonomy created by NANDA, nursing diagnoses related to impaired gas exchange, pain, rest, skin integrity, and problems related to gynecologic-obstetric aspects were established.

Planning

The objectives and interventions were established based on the previously marked diagnoses, using the NOC and NIC taxonomy. Those related to gas exchange, skin care at the level of the chest tube area, pain, rest and those with gynaecological links stand out.

Discussion

CN is a rare entity and it is necessary to know its etiopathogenesis for early detection of the problem and apply care aimed at meeting the needs of the patient and avoiding complications derived from it.

Introducción

El neumotórax catamenial (NC) es un tipo de neumotórax poco frecuente que afecta a mujeres en edad fértil. Su etiopatogenia es controvertida y son varias las teorías propuestas, aunque existe una relación frecuente con la endometriosis intratorácica. El diagnóstico puede resultar un desafío por lo que pueden aparecer recurrencias hasta dar con éste y poder tratarlo correctamente. Este documento describe un caso de NC y elabora un plan de cuidados orientado a una paciente que sufre este tipo de patología.

Descripción del caso

Mujer de 41 años, sin antecedentes personales ni familiares que acude al servicio de urgencias refiriendo dolor en hemitórax derecho y disnea. En la radiografía de tórax se evidencia un neumotórax completo que requirió una colocación de drenaje torácico. Durante la intervención quirúrgica se observan lesiones cicatriciales con poros diafragmáticos de color gris y negro, lo que hace vislumbrar un neumotórax de origen catamenial.

Valoración

La valoración de enfermería se diseñó mediante el modelo conceptual de Marjory Gordon, donde se identificaron como alterados los patrones de actividad-ejercicio, descanso, el perceptivo y el relacionado con la sexualidad.

Diagnósticos

Mediante la taxonomía creada por la NANDA se establecieron los diagnósticos enfermeros relacionados al deterioro del intercambio gaseoso, dolor, descanso, integridad de la piel y los problemas vinculados con aspectos gineco-obstétricos.

Planificación

Los objetivos e intervenciones se establecieron a partir de los diagnósticos previamente marcados, utilizando la taxonomía NOC y NIC. Se destacan los relacionados con el intercambio gaseoso, los cuidados de la piel a nivel de la zona del tubo torácico, dolor, descanso y aquellos con vinculación ginecológica.

Discusión

El NC es una entidad poco frecuente y es necesario conocer su etiopatogenia para una detección precoz del problema y aplicar unos cuidados orientados a satisfacer las necesidades de la paciente y a evitar complicaciones derivados de ésta.
