A 48-year-old male patient came to our consultation due to neuropathic pain in the lower left extremity and a mass in the ipsilateral gluteus. MRI showed a lipoma of the pelvis measuring 20cm in diameter passing through the greater sciatic notch with displacement of adjacent structures (Figs. 1 and 2). Complete local resection was performed. Histopathology revealed a hibernoma weighing 980g.

Hibernomas are a rare variation of lipoma associated with a specific cytogenetic alteration. They may be found in diverse locations, creating increased volume and compressive symptoms. Complete resection is indicated in symptomatic cases or those with uncertain diagnosis. They have no potential for malignancy or recurrence.
