Los neuroestimuladores periféricos se pueden emplear para el tratamiento del dolor neuropático, y su uso ha aumentado al emplearse la ecografía para su colocación. La neuropatía del nervio radial superficial es frecuente y se han descrito implantes a nivel humeral distal con adecuado control del dolor, pero no todos los pacientes presentan un bloqueo test positivo en este lugar, necesario para su implante. Presentamos un caso de implante ecoguiado de neuroestimulador periférico en el canal de torsión radial como tratamiento de una neuropatía del nervio radial superficial con adecuado control del dolor durante más de un año, aunque tras ese tiempo el electrodo sufrió una fractura y tuvo que retirarse y reimplantarse de forma quirúrgica. Este enfoque puede ser una alternativa eficaz en pacientes seleccionados, aunque se requieren más estudios.
Peripheral neurostimulators may be used for treatment of neuropathic pain, and their use has increased with the use of ultrasound for its placement. Superficial radial nerve neuropathy is common, and distal humeral implants have been described with adequate pain relief, but not all patients present a positive test block in this site, which is needed for the implant. We present a case of ultrasound-guided implant of a peripheral neurostimulator in the radial spiral groove as treatment of superficial radial nerve neuropathy with adequate pain relief, although after one year the electrode suffered a fracture and had to be removed and reimplanted surgically. This new technique may be an effective alternative in selected patients, but further studies are needed.
