The video shows the surgical treatment of Zenker's diverticulum of the oesophagus, assisted by intraoperative endoscopy. This is performed with a left transcervical approach.(Fig. 1)

Once in the left paraesophageal and prevertebral space, dissection of the diverticulum and its neck takes place, and with the support of intraoperative endoscopy, the diverticulum is resected using an endograft. Subsequently, myotomy of the cricopharyngeus muscle is then undertaken, extending craniocaudally into the pharyngoesophageal musculature.

Intraoperative endoscopy verifies the correct and complete extension of the myotomy, as well as the absence of perforation and adequate calibre of the oesophageal lumen.
