Abstract
Introduction

Readmission to the Intensive Care Unit (ICU) determines worse outcomes such as higher mortality, increased hospital and ICU stay, as well as higher economic costs. When deciding which patient is suitable for transfer from the ICU, factors associated with readmission must be considered to avoid it. Knowledge of these factors helps professionals identify those patients with a higher probability of readmission, prioritizing their care, establishing and preparing interventions that seek to reduce the risk of readmission.

Objective

Determine factors associated with early readmission in patients transferred from the ICU to general hospitalization wards of the same hospital.

Method

Studies were retrieved from databases: CINAHL, EMBASE, BVS, PubMed, SCOPUS and WOS identifying original studies on adult patients readmitted early to the ICU during the same hospitalization, in any language and without time limit. Studies of patient readmission after seven days, review articles, editorials, protocols, clinical guidelines, qualitative studies and opinion surveys were excluded.

Results

Of 755 files found, 28 articles made up the review. The most analyzed factors were age, sex, severity of the disease, comorbidity, length of stay in the ICU, mechanical ventilation and nocturnal discharge. Those most frequently associated with readmission were age, severity of illness, comorbidity, and length of ICU stay. NEWS, MEWS, and SWIFT scores were also factors associated with readmission.

Conclusion

More research is needed to identify those modifiable factors that can decrease readmission rates. Using readmission prediction instruments at the time of discharge could support the decision of which patient is most prepared for it.

Introducción

El reingreso a la Unidad de Cuidados Intensivos (UCI) determina peores resultados como mayor mortalidad, aumento de estancia hospitalaria y de UCI, además mayores costos económicos. Al momento de decidir qué paciente está en condiciones de transferirse de UCI, se deben considerar factores asociados a la readmisión para evitarla. El conocimiento de esos factores ayuda a los profesionales a identificar aquellos pacientes con mayor probabilidad de readmisión, priorizando su atención, estableciendo y preparando intervenciones que busquen disminuir el riesgo de reingreso.

Objetivo

Determinar los factores asociados a la readmisión temprana en pacientes transferidos de UCI a salas generales de hospitalización del mismo hospital.

Método

Los estudios fueron recuperados de bases de datos: CINAHL, EMBASE, BVS, PubMed, SCOPUS y WOS identificando estudios originales sobre pacientes adultos readmitidos tempranamente en la UCI durante la misma hospitalización, en cualquier idioma y sin límite de tiempo. Se excluyeron los estudios de readmisión de pacientes posterior a siete días, artículos de revisiones, editoriales, protocolos, guías clínicas, estudios cualitativos y encuestas de opinión.

Resultados

De 755 archivos encontrados, 28 artículos conformaron la revisión. Los factores más analizados fueron edad, sexo, gravedad de la enfermedad, comorbilidad, duración de la estancia en UCI, ventilación mecánica y alta nocturna. Los asociados con mayor frecuencia con reingreso fueron edad, gravedad de la enfermedad, comorbilidad y duración de la estancia en UCI. Las puntuaciones de la National Early Warning Score (NEWS), Modified Early Warning Score (MEWS) y Stability Workload Índex for Transfer (SWIFT) también fueron factores asociados con la readmisión.

Conclusión

Se hace necesario realizar más investigaciones para identificar aquellos factores modificables que pueden disminuir las tasas de reingreso. El utilizar instrumentos de predicción de reingreso al momento del alta podría apoyar la decisión de qué paciente está más preparado para ello.
