Introduction and case presentation

Kounis Syndrome is the association of acute coronary syndrome (ACS) in the context of a hypersensitivity reaction whose prevalence is difficult to identify and its etiology is multifactorial, which makes its early detection difficult. The unusual case of a woman who suffers an acute myocardial infarction (AMI) with cardiorespiratory arrest (CPA) secondary to the accidental sting of a bee is presented. The objective is, regarding the case, to define the nursing interventions and the individualised care plan.

Assessment

Based on Virginia Henderson's care model, the assessment of the nursing care process was carried out.

Diagnosis and planning

According to the NANDA taxonomy, the nursing diagnoses and potential problems that developed during their stay in the unit were described. Standardized language was used establishing NOC objectives and NIC interventions.

Discussion and conclusions

The combination of both pathologies in a single entity can make the management of this syndrome in the acute phase difficult, which modifies the conventional scenario of a PCR. Defining a standardised care plan is a fundamental axis to provide optimal quality of care based on evidence in the absence of standardised algorithms in the literature.

Introducción y presentación del caso

El Síndrome de Kounis es la asociación de un síndrome coronario agudo (SCA) en el contexto de una reacción de hipersensibilidad cuya prevalencia es difícil de filiar y su etiología es multifactorial, lo que dificulta su detección temprana. Se presenta el inusual caso de una mujer que sufre un infarto agudo de miocardio (IAM) con parada cardiorrespiratoria (PCR) secundario a la picadura fortuita de una abeja. El objetivo es, a propósito del caso, definir las intervenciones de enfermería y el plan de cuidados individualizado.

Valoración

A partir del modelo de cuidados de Virginia Henderson se realizó la valoración de enfermería tras el tratamiento de reperfusión urgente.

Diagnóstico y panificación

Según la taxonomía NANDA se describieron los diagnósticos enfermeros y problemas potenciales que se desarrollaron durante su estancia en la unidad. Se utilizó lenguaje estandarizado estableciendo objetivos NOC e intervenciones NIC.

Discusión y conclusiones

La combinación de ambas patologías en una sola entidad puede dificultar el manejo de este síndrome en fase aguda que modifica el escenario convencional de una PCR. Definir un plan de cuidados normalizado supone un eje fundamental para proporcionar una calidad asistencial óptima y basada en la evidencia ante la ausencia bibliográfica de algoritmos estandarizados.
