Abstract

Airway pathology is a complex and incompletely mastered field. Historically, its management was rudimentary, with tracheostomies performed in ancient Egypt, and progress remained stagnant for millennia. Significant advances began in the late 19th century, followed by notable surgical and anesthetic progress in the mid-20th century. The 1952 polio epidemic spurred further development, with tracheal resection and anastomosis as the first major milestone, later extending to the larynx and carina. All significant advances in airway surgery ended in the latter half of the 20th century, with minimal progress since. This article analyzes its atypical evolution of an initial step that awaited millennia for subsequent advances, only to halt again in recent times.

La patología de la vía aérea es un campo complejo y no completamente dominado. Históricamente, su manejo ha sido rudimentario, con traqueostomías realizadas ya en el antiguo Egipto, permaneciendo su progreso estacionario por milenios. Los avances más significativos se iniciaron a finales del siglo XIX y con el notable progreso de la cirugía y la anestesia a mediados del siglo XX. La epidemia de polio de 1952 le dio su gran empujón, con la resección traqueal y anastomosis como piedra angular del mismo, extendiéndose posteriormente a laringe y carina. Todos los avances significativos en la cirugía de la vía aérea finalizaron a finales del siglo XX, con un progreso mínimo desde entonces. Este artículo analiza esta atípica evolución con un paso inicias que esperó durante milenios para siguientes avances, para volver a parar en tiempos más recientes.
