A 74-year-old woman presented with chest pain, dyspnea, and swelling of the neck. Using a cervical Kocher incision and median sternotomy, a cervical thyroid lesion and mediastinal tissue were resected. Intraoperatively, no connection was observed between the cervical thyroid tissue and the mediastinal lesion. Histopathologically, the cervical thyroid tissue was diagnosed as papillary carcinoma, while the mediastinal lesion was diagnosed as benign thyroid tissue. A coronal thoracic CT scan (Fig. 1) and resected materials (Fig. 2) showed papillary thyroid carcinoma (yellow stars) and giant primary ectopic mediastinal thyroid tissue (red stars).
