Abstract
Objectives

The aim of the study is comparing endoscopic coblation adenoidectomy and suction diathermy in terms of surgical duration, intraoperative blood loss, and postoperative complications such as foul odor, neck pain, and secondary bleeding, as well as patient recovery.

Patients and methods

Prospective randomized double blinded controlled trial involving 210 patients who presented with adenoid hypertrophy. Randomization was done in two groups group A including 105 patients who underwent endoscopic adenoidectomy by suction diathermy, Group B including 105 patients who underwent endoscopic adenoidectomy by coblation technique.

Patients were compared regarding intraoperative blood loss and operative time and postoperative bleeding, postoperative neck pain, and halitosis.

Results

When comparing the two groups, Group B had notably longer surgeries but experienced less pain after the operation, recovered more quickly, and had a lower chance of bad breath. There was no significant difference between both the groups regarding intraoperative blood loss.

Conclusion

Both techniques are effective in management of adenoid hypertrophy, but results showed endoscopic adenoidectomy with coblation is favored over suction diathermy due to better postoperative outcomes.

Objetivos

El objetivo del estudio es comparar la adenoidectomía endoscópica por coblación y la diatermia por succión en términos de duración quirúrgica, pérdida hemática intraoperatoria y complicaciones postoperatorias como mal olor, dolor cervical y sangrado secundario, así como la recuperación del paciente.

Pacientes y métodos

Ensayo clínico prospectivo, aleatorizado, doble ciego y controlado, con 210 pacientes con hipertrofia adenoidea. La aleatorización se dividió en dos grupos: el grupo A, con 105 pacientes sometidos a adenoidectomía endoscópica mediante diatermia por succión, y el grupo B, con 105 pacientes sometidos a adenoidectomía endoscópica mediante la técnica de coblación.

Se compararon los pacientes en cuanto a pérdida hemática intraoperatoria, tiempo operatorio, sangrado postoperatorio, dolor cervical postoperatorio y halitosis.

Resultados

Al comparar los dos grupos, el Grupo B tuvo cirugías notablemente más largas, pero experimentó menos dolor posoperatorio, se recuperó más rápido y tuvo menor probabilidad de mal aliento. No se observaron diferencias significativas entre ambos grupos en cuanto a la pérdida de sangre intraoperatoria.

Conclusión

Ambas técnicas son eficaces en el tratamiento de la hipertrofia adenoidea, pero los resultados mostraron que la adenoidectomía endoscópica con coblación es preferible a la diatermia por succión debido mejores resultados postoperatorios.
