In the pre-surgical studies, blood count showed as an incidental finding, total leukocyte count of 15,960 with 32% of atypical cells as the only abnormality. Upon suspicion of malignancy, bone marrow biopsy was performed, finding various cellularity with intrasinusoidal infiltration of CD20+ cells. For this reason, <span class="elsevierStyleSup">18</span>F-FDG PET/CT was performed, finding nodal (aortocaval and inguinal lymph nodes) and extra nodal (renal) lymphoma involvement. Treatment was initiated, and follow-up will be carried out. (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0010" class="elsevierStylePara elsevierViewall">Secondary extra nodal involvement can be found in up to 30% of patients with non-Hodgkin lymphoma. After bone marrow and reticuloendothelial organs, the kidneys are the most frequently affected organs; typically, by B-cell type tumors (intermediate and high grade) or Burkitt lymphoma. Although renal involvement has been reported in up to 37.6% of autopsies, it is rarely symptomatic and only 5%–8% of cases show imaging abnormalities<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a>.</p><p id="par0015" class="elsevierStylePara elsevierViewall">Considering that the kidney is an organ that lacks lymphatic tissue, it is believed that the compromise arises from the renal capsule or the perirenal fat and subsequently spreads into the parenchyma. Therefore, there are multiple patterns of presentation, being the less frequent (<10%) the one we show in this case, where there is diffuse infiltration of the perirenal fat. Other imaging patterns, almost invariably accompanied by enlarged retroperitoneal lymph nodes, include: i) multiple parenchymal or renal sinus masses (60%), ii) a solitary retroperitoneal mass with kidney invasion (30%), iii) a solitary renal mass (20%), and iv) diffuse infiltration of the renal parenchyma (<20%)<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a>.</p><p id="par0020" class="elsevierStylePara elsevierViewall"><span class="elsevierStyleSup">18</span>F-FDG PET/CT plays a fundamental role in these patients since it can be the only clue to detect renal involvement which, as mentioned, is usually clinically silent. For this reason and considering the increasing use of <span class="elsevierStyleSup">18</span>F-FDG PET/CT as part of the initial staging of lymphoma patients, it is important to know the different possible presentation patterns.
