The secondary objective is to attempt to identify factors related to the onset of cancer in this patient cohort.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We carried out a retrospective cohort study based on information extracted from the database of ICD carriers of the Hospital Complex of Navarra (<span class="elsevierStyleItalic">Complejo Hospitalario de Navarra</span>) in Spain.</p><p id="par0030" class="elsevierStylePara elsevierViewall">All patients who had an ICD implanted at this site between January 2000 and January 2017 were included in the study sample. Oncological and mortality data of this patient cohort were collected and updated until 30 June 2018. All information was extracted from the computerized medical records system of the Navarra Health Service (<span class="elsevierStyleItalic">Servicio Navarro de Salud)</span>, and subsequently recorded in a coded and encrypted database.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Living patients were asked to sign an informed consent form authorizing their follow-up. In the case of deceased patients, given the clinical relevance of this study and the fact that having access to information from this group of patients was considered essential for the research, a waiver of consent was obtained from the competent authorities listed below. The present study was granted approval from the Clinical Research Ethics Committee of Navarra and the Health Department of the Regional Government of Navarra (<span class="elsevierStyleItalic">Departamento de Salud del Gobierno de Navarra</span>).</p><p id="par0040" class="elsevierStylePara elsevierViewall">Data on the patients’ sociodemographic variables; comorbidities, including risk factors for the onset of cancer; type of heart disease warranting the implantation of an ICD; oncological disease; and mortality were collected. The oncological diagnosis is based on the pathology study results reported in the patients’ medical records.</p><p id="par0045" class="elsevierStylePara elsevierViewall">The patients’ sociodemographic and clinical characteristics were analyzed. Throughout the follow-up period, cumulative incidence rates (CIRs) of cancer were estimated both for the pooled sample and for particular subgroups. These cancer CIRs were compared with population data from 2015<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> drawn from the Spanish Network of Cancer Registries (REDECAN, <span class="elsevierStyleItalic">Red Española de Registros de Cáncer</span>), as well as with data from a cohort of patients with heart failure and similar demographic characteristics (age and sex) to those of our patient cohort, but most of whom were not ICD carriers.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> For the comparison with the REDECAN data,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> incidence rates were adjusted according to the patients’ age and sex based on the population data provided by the Spanish National Institute of Statistics (INE, <span class="elsevierStyleItalic">Instituto Nacional de Estadística de España</span>).<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Finally, univariate and multivariate models were developed to identify variables associated with the onset of cancer following an ICD implantation, considering variables that might falsely reduce the incidence of neoplasms by causing a patient’s premature death (terminal heart failure, arrhythmic death syndrome, stroke, infections, or other non-oncologic causes of death).</p><p id="par0055" class="elsevierStylePara elsevierViewall">The following software programs were used to perform the statistical analyses: SPSS Statistics v23, R v4.0.1, and OpenEpi. The <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> or its non-parametric analog (i. e., Fisher’s exact test) was used to compare categorical variables (proportions), and a <span class="elsevierStyleItalic">X</span><span class="elsevierStyleSup">2</span> linear trend test was used for ordinal variables. An analysis of variance (ANOVA), Student’s T-test, or its non-parametric analog (i.e., the Wilcoxon-Mann-Whitney test) was used to compare continuous variables when the normality of the dependent variable could not be assumed. Finally, an analysis based on the Fine-Gray competing risks model was developed to identify variables associated with the onset of neoplasms during the follow-up period.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Participants</span><p id="par0060" class="elsevierStylePara elsevierViewall">Data from a total of 497 patients who met the eligibility criteria of having had an ICD implanted in the Hospital Complex of Navarra throughout the screening period were included in the study. No patients were lost to follow-up, and data of all patients were analyzed.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Baseline and follow-up characteristics of the study sample</span><p id="par0065" class="elsevierStylePara elsevierViewall">As shown in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>, the patients included in our cohort had a mean age of 59.98 (14.98) years, 89.1% were men, 67.6% had been smokers, 12.5% were active smokers, and 15.5% of them had a diagnosis of chronic obstructive pulmonary disease (COPD). Over half of the patients (51.7%) consumed alcoholic beverages, with this consumption being “high” (≥70 g/week in men and ≥50 g/week in women) in 18.2% of cases.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0070" class="elsevierStylePara elsevierViewall">The cardiac disease warranting the need for the implantation of a defibrillator was ischemic heart disease in 51.6% of cases and non-ischemic dilated cardiomyopathy (DCM) in 28.2% of cases, with other types of heart conditions being less prevalent.</p><p id="par0075" class="elsevierStylePara elsevierViewall">Prior to the implantation of the ICD, 8.9% of the patients had a history of an oncological disease, with the most frequent tumors being prostate, bladder, colon, pharyngeal/laryngeal, and hematological ones (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0080" class="elsevierStylePara elsevierViewall">There were some missing data for certain secondary variables, but in no case did this loss of data account for over 5% of information for a specific variable. There were no missing data for variables of greater interest, such as those relating to cancer or mortality. Hence, no data imputation methods were applied.</p><p id="par0085" class="elsevierStylePara elsevierViewall">The mean follow-up period of the study cohort was 85 (44.77) months.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Mortality data</span><p id="par0090" class="elsevierStylePara elsevierViewall">During the follow-up period, 35% of the patients died at a mean age of 71.93 (10.53) years. The most frequent cause of death was heart disease (46%), with death secondary to heart failure (39.1%) being more frequent than that caused by arrhythmias (6.9%). The second most frequent type of etiology was oncological disease (16.1) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Oncological disease data of the study sample</span><p id="par0095" class="elsevierStylePara elsevierViewall">During the follow-up period, 15.3% of the patients developed neoplasms following the ICD implantation, with 60.8% of cases being related to tobacco consumption.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> These tumors were responsible for the death of approximately 5.6% of all patients. The mean time elapsed between the ICD implantation and the onset of the oncological disease was 4.84 (3.17) years. The most frequent tumors were mostly similar to those developed before the ICD implantation (prostate, bladder, colon, and pharyngeal/laryngeal), except for the higher incidence of lung cancer and the lower incidence of hepatobiliary-pancreatic and hematological neoplasms (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">The age-adjusted CIR of cancer in our study sample with respect to the reference population was 1230.9 per 100,000 person-years. This rate is significantly higher (<span class="elsevierStyleItalic">P</span> < .001) among smokers (1382.2 per 100,000 person-years) or patients with ischemic heart disease (1563.3 per 100,000 person-years), and significantly lower (<span class="elsevierStyleItalic">P</span> < .001) among non-smokers (587.5 per 100,000 person-years) or patients with heart disease of non-ischemic etiology (666.3 per 100,000 person-years).</p><p id="par0105" class="elsevierStylePara elsevierViewall">The population CIR reported by the REDECAN in 2015<span class="elsevierStyleSup">12</span> (653.1 cases per 100,000 person-years) is significantly lower (<span class="elsevierStyleItalic">P</span> < .001) than the overall age-adjusted CIR calculated for our study sample and that of higher-risk subgroups, such as smokers or patients with an ischemic heart disease, but similar to that of non-smokers or patients without an ischemic heart disease.</p><p id="par0110" class="elsevierStylePara elsevierViewall">The recently published CIR for a Spanish cohort of patients with heart failure associated with a reduced left ventricular ejection fraction (LVEF), of whom only 20.4% were ICD carriers, is 1288.6 cases per 100,000 person-years.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> This rate is very similar to that found in our pooled population of ICD carriers.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Variables associated with the onset of cancer after an ICD implantation</span><p id="par0115" class="elsevierStylePara elsevierViewall">Variables found to be significantly associated with the onset of cancer following an ICD implantation, after adjusting the data for causes of death other than oncological disease following an ICD implantation that might compete with the growth of neoplasms, are older age (sub-distribution hazard ratio [sHR] 1.04; 95% confidence interval [CI] 1.02−1.06), higher daily cigarette consumption (sHR 1.02; 95% CI 1.00−1.03), and ischemic heart disease (sHR 1.22; 95% CI 1.12−1.34) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>).</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Discussion</span><p id="par0120" class="elsevierStylePara elsevierViewall">Several research groups have proposed a potential cause-effect relationship between ICDs and the onset of cancer.<a class="elsevierStyleCrossRefs" href="#bib0045"><span class="elsevierStyleSup">9,10</span></a> Our hypothesis is that said relationship mainly results from the coexistence of certain risk factors and markers associated with the growth of neoplasms and certain heart diseases, and that this could explain the higher incidence of cancer among patients with an ICD.</p><p id="par0125" class="elsevierStylePara elsevierViewall">In this regard, as shown by the data obtained in the present study, as well as in registries of ICD carriers<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> or of patients with heart failure associated with a reduced LVEF,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> approximately half of these patients have a personal medical history of ischemic heart disease, which is strongly associated with older age, male sex, and a high prevalence of smoking. A high rate of deaths was also observed during the follow-up period of our study, which falls in line with the mortality data reported in other studies including patients with generally advanced heart diseases.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18,21,22</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">When we analyzed the oncological data of the patients included in our study sample, the high CIR of cancer stood out, as it was much higher than that of the population used as a comparator,<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> but similar to that observed in a cohort of Spanish patients with heart failure and a reduced LVEF, most of whom were not carriers of an ICD.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In contrast, when we stratified the sample according to the patients’ history of smoking and ischemic heart disease, the CIR of the non-smokers and non-ischemic patients were very similar to those of the general population.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> This relationship was confirmed in the univariate and multivariate models adjusted for competing risks, as their findings showed a higher oncological risk among older patients, smokers, and those with ischemic heart disease.</p><p id="par0135" class="elsevierStylePara elsevierViewall">These observations suggest that certain characteristics (advanced age, smoking, etc.) that tend to be more prevalent among cohorts of ICD carriers<a class="elsevierStyleCrossRefs" href="#bib0050"><span class="elsevierStyleSup">10,20</span></a> or patients with heart failure associated with a reduced LVEF,<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> may be the main factors responsible for the higher incidence of cancer in these groups of patients, which would support our hypothesis linking the higher incidence of cancer among patients with ICDs to these factors, rather than to the ICD itself.</p><p id="par0140" class="elsevierStylePara elsevierViewall">Extrapolation of the results of this study to the general population seems reasonable considering that the source of our data was the database of the only designated tertiary hospital of the Autonomous Community of Navarra, which, therefore, services the vast majority of this population, regardless of their age, sex, or socioeconomic status.</p><p id="par0145" class="elsevierStylePara elsevierViewall">The main limitation of our study is the lack of patients without an ICD in our sample, which prevented us from performing direct comparisons and led to the need to obtain these data from external registries.<a class="elsevierStyleCrossRefs" href="#bib0085"><span class="elsevierStyleSup">17,18</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">Another limitation inherent to this study is the small cohort size and the cumulative follow-up period, as an increased number of cases of cancer could have been detected if they had both been greater.</p></span><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Conclusions</span><p id="par0155" class="elsevierStylePara elsevierViewall">Patients with an ICD have a higher incidence of cancer than the general population, albeit similar to that of a contemporary cohort of heart failure patients with a reduced LVEF, most of whom do not have an ICD. Tobacco consumption and older age seem to significantly influence the growth of neoplasms. Likewise, some investigations describe a higher incidence of cancer in patients with an ICD that suggest a potential relationship.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0070" class="elsevierStyleSimplePara elsevierViewall">Retrospective cohort study of patients that underwent implantation of an ICD in the <span class="elsevierStyleItalic">Complejo Hospitalario de Navarra</span> between 2000 and 2016. The follow-up finished in June 2018.</p><p id="spar0075" class="elsevierStyleSimplePara elsevierViewall">Sociodemographic, comorbidities and oncological data was analysed. Cancer incidence rates were estimated and compared with the general population data and with data of a cohort of patients with reduced LVEF heart failure. Risk of cancer models were adjusted by competitive risk models.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0080" class="elsevierStyleSimplePara elsevierViewall">497 patients were included, mostly male (89.1%), with a mean age of 59.98 (14.98) years and a proportion of smokers of 67.6% and of ischaemic heart disease of 51.6%. The cancer incidence rate in the sample was 1230.9 per 100.000 person-year.
