One of the most recent forms of synthesis of the literature are the scoping reviews, which show a spectacular growth in number of publications: more than 1000 articles published in 2018 in MedLine while, in 2010, there were only 50 published. Almost 75% of scoping reviews address health-related issues2 and, at present, updates or methodological articles on knowledge about scoping reviews continue to be published, its objectives continue to be subject of debate and a standard methodology continues being sought.3–5

These publications are but the consequence and/or need to reduce uncertainty for authors and readers regarding scoping reviews. The translation of this term into the Spanish language leads to the use of different terminologies, such as “scoping reviews”, “reviews on the scope of evidence”, “exploratory systematic reviews”, etc.; in view of the disparity, the term scoping review has been incorporated in the title of the articles published in Spanish. In the absence of consensus, in this article we chose to use the term “Panoramic Review” (literal translation) to refer to a Scoping Review (ScR).

The lack of uniformity around ScR also reaches its objectives, methodology and applicability.7 Levac collects up to 7 different definitions and objectives for ScR according to different authors.8 From “a preliminary evaluation of the potential volume and scope of the literature to be researched” to “exploratory studies that systematically identify the available literature on a subject” or “review to identify key concepts, theories and deficiencies in research”. The debates and/or inconsistencies around ScR can be summarized in the following questions. In the continuum of the literature review spectrum: Where are the ScR positioned? What are its limits? What are its fields of applicability? Can we call them exploratory reviews? Systematic? and the most recent adjective, Fast?

For some authors, a ScR is considered as part of a continuous review process whose ultimate goal is to perform a complete systematic review (SLR). For many others, a ScR is perceived as a method in itself, leading to the publication and dissemination of research results in a particular subject. ScR9 are rare in Spain, most of them are published in international journals10 and few of them address the aforementioned methodological debates. This article aims to bring readers closer to ScR, their strengths and weaknesses, mainly analysing the differences between systematic literature reviews (SLR) and ScRs. Table 1 summarizes those differences including the “narrative review”.
<elsevierMultimedia ident="tbl0005"></elsevierMultimedia>Differences in questions and/or objectives
Unlike SLRs, which address “specific questions” in “specific populations”, ScR allow us to study not only “aetiologies”, “risk factors” or outcomes in the studies, but aspects related to the design, with the frameworks or theoretical models, in certain aspects of the populations, etc. It can thus raise more complex or different issues whose response goes beyond the simple consideration of what is or what is not effective.11 The formulation of the question has a more qualitative “aspect”, a consequence of more complex and multidimensional questions, compatible with the current state of knowledge. Thus, ScR may have different objectives: to make a map of the key concepts that sustain a research area; clarify concept definitions and/or the conceptual limits of a subject; differentiate according to designs; according to methodological approaches (quantitative and/or qualitative). A ScR can focus on one or both of these objectives as a whole.4 Examples of possible ScR questions are included in Table 2.
<elsevierMultimedia ident="tbl0010"></elsevierMultimedia>Differences in applicability
Another differentiating and controversial point in relation to the ScR is the consideration that these studies are only applicable to more emerging knowledge areas versus knowledge areas with greater evidence, which should be approached with a SLR.4 In this sense, ScR could help to identify the appropriate parameters of a review (define the target population, intervention, comparison groups and/or outcomes), all known as “PICO strategy” to identify the possible volume of a SLR and the cost of it. But, depending on the objectives that are formulated, ScR can address areas with a greater wealth of literature, but very fragmented and diverse, or very focused on specific aspects. This will be the case of ScR that address the review of SLR (scoping reviews on systematic literature reviews).12

On the other hand, ScR tend to have a less clinical and more sociological approach, helping future research rather than helping with current treatments or policies, because, among other things, its methodology places little emphasis on the methodological quality of the studies.8 However, the broader approach of the information to be sought, does allow to make certain types of decisions, for example: which research area or which population should benefit from more resources.
Differences in methodologies
The first methodological development proposal regarding ScR was made in 2005 by Arksey and O’Malley13 describing 6 stages: [1] identification of research questions: generally broad; [2] identification of relevant studies: as broad as possible; [3] selection of studies: establishing inclusion/exclusion criteria; [4] mapping of data: mapping and classification of information according to the main issues and subjects; [5] collect, summarize and report the results: descriptive and/or numeric summary of the data and a thematic analysis, and [6] expert consultation exercise: optional additional step to involve key stakeholders and validate the results of the study. Subsequently, Levac8 and the Joanna Briggs Institute11 made contributions to each of the stages to improve the clarity and rigour of the review process, for example: expansion of the inclusion criteria (types of participants, concepts or context), standardized data extraction, data charting (show the results by means of graphs, according to the inclusion criteria) and implications for practice, research and/or policies.

ScR are more flexible in terms of methodology, since it can be reviewed during the process according to the outcomes found (e.g., according to homogeneity) and the researchers’ own criteria, and it is common to consider different types of studies (e.g., interventional, observational and qualitative in the same review). In general, these reviews allow synthesizing information from a quantitative, qualitative or mixed point of view,3 although the quantitative synthesis of the data and the evaluation of the methodological quality of the studies is more the exception than the rule.4

“Stage 6” is a specific stage of ScR that was initially considered optional but now recommended by some authors to be mandatory, although most published ScR do not include the recommended expert consultation.
Guidelines for making and publishing scoping reviews
Given the existing controversies and unawareness, different guidelines have been published in recent years to prepare and publish a ScR. Table 2 shows a series of guidelines whose reading is recommended before starting a ScR.

In 2011 the Cochrane Public Health Group14 gave differentiating guidelines between an SLR and an ScR, directing the latter towards a first step in performing a SLR. The review by Pham et al. on ScR concluded that these differed not only in terms of the objective or methodologies, but also in the detail of the reports and/or articles.2 This is why the Equator network has recently published an extension of the PRISMA checklist to include ScR (PRISMA-ScR).15 This checklist can serve as a detailed guide for researchers who plan to perform a ScR.

The truth is that with the publication of this checklist, it is intended that ScR can be reproduced without losing its flexibility (e.g.: the search strategy must be described, and the decisions made must be stated/justified). It also raises the possibility of incorporating a methodological quality assessment if this makes sense given the research question, which leaves the possibility of performing more powerful ScR, with more direct practice and policy implications.

However, this methodological rigour and the exhaustiveness of the report move ScR away from “fast reviews” that aim to produce immediate recommendations to inform policy makers and others involved.16 There are several methodologies in terms of fast reviews, but what they have in common is the possibility of skipping some parts of the rigorous SLR protocol in order to provide the fastest information.17 However, a ScR on an emerging issue close in time (e.g. Zika virus) may require a literature search that covers a small number of years and therefore could respond to this urgent need in a fast and rigorous way.

Although it may seem otherwise, a good ScR may require more time, coordination and/or effort to complete than a SLR, because of the broader research questions and the corresponding larger number of articles that will need to be reviewed; for the need/opportunity to adapt the protocol based on the outcomes and, lastly, for having to consult with an expert panel.
Conclusions
ScR can be of various types and perhaps that is why there is no clear and comprehensive definition. ScR can be more complex than SLRs since they can have different objectives and methodologies and the team's good judgement in choosing the methodology is even more relevant.
