The most common variant is myxoid, with the pleomorphic variant being the most aggressive.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a></p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Clinic case report</span><p id="par0010" class="elsevierStylePara elsevierViewall">Male, 46, exhibiting painless right proptosis with 18 months evolution (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>). Patient denied diplopia.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0015" class="elsevierStylePara elsevierViewall">Examination revealed a visual acuity of 20/20 in both eyes, exophthalmometry in RE 22<span class="elsevierStyleHsp" style=""></span>mm, LE 15<span class="elsevierStyleHsp" style=""></span>mm. IOP RE of 12, LE 13<span class="elsevierStyleHsp" style=""></span>mmHg. Right eye ocular fundus with retinochoroidal folds.</p><p id="par0020" class="elsevierStylePara elsevierViewall">Computerized axial tomography (CAT) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>) and nuclear magnetic resonance (NMR) (<a class="elsevierStyleCrossRef" href="#fig0015">Fig. 3</a>) were requested.</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><elsevierMultimedia ident="fig0015"></elsevierMultimedia><p id="par0025" class="elsevierStylePara elsevierViewall">Incisional biopsy (<a class="elsevierStyleCrossRef" href="#fig0020">Fig. 4</a>) reported well-differentiated liposarcoma (<a class="elsevierStyleCrossRef" href="#fig0025">Fig. 5</a>). One month later, exenteration was performed and the patient treated with 33 radiotherapy sessions at a dose of 66<span class="elsevierStyleHsp" style=""></span>Gy, rehabilitating the patient with orbital prosthesis (<a class="elsevierStyleCrossRef" href="#fig0030">Fig. 6</a>).</p><elsevierMultimedia ident="fig0020"></elsevierMultimedia><elsevierMultimedia ident="fig0025"></elsevierMultimedia><elsevierMultimedia ident="fig0030"></elsevierMultimedia><p id="par0030" class="elsevierStylePara elsevierViewall">To date, the patient remains alive 4 years after initial diagnosis. Local relapse or systemic extensions have not been found (documented with positron emission tomography, PET-TC).</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0055">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">Liposarcoma is an adipose tissue malign tumor, probably developed from mesenchymal cells instead of pre-existing lipoma.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> Until 2008,<a class="elsevierStyleCrossRef" href="#fn0005"><span class="elsevierStyleSup">1</span></a> 37 cases of primary orbit liposarcoma have been published in the literature.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">12</span></a></p><p id="par0040" class="elsevierStylePara elsevierViewall">Liposarcoma is the most common soft tissue sarcoma in adults,<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> accounting for 20% of all cases.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">8</span></a> However, this sarcoma is rare in the orbit and the majority are primary, accounting for less than 1% of orbit biopsies.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">9</span></a> On the basis of published clinic case reports, mean onset age has been estimated at 42.9 years. The review by Madgeet al.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">12</span></a> referred predominance in females (64.9%) of primary orbit liposarcoma. The present review found a very similar mean onset age (44.75 years) with a clear predominance of 75% in males (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). It is characterized by proptosis, ocular dystopia, diplopia, pain and optic nerve compression depending on location.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> The duration of symptoms varies between 2 weeks and 7 years.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">8</span></a></p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0045" class="elsevierStylePara elsevierViewall">There are 5 sub-types of liposarcoma: well-differentiated<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> (30% of orbit cases), myxoid (the most common type, representing 60% of orbit cases), round cell, poorly differentiated and pleomorphic<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> (5% of cases and the worst prognostic<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">8</span></a>). Well-differentiated and myxoid types have a benign clinic course, with the round cell and pleomorphic type being more aggressive.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a> The well-differentiated type is also known as “atypical lipomatous tumor”.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">10</span></a></p><p id="par0050" class="elsevierStylePara elsevierViewall">Inflammatory cells have been described in liposarcoma, and accordingly it is believed that some cases can have an inflammatory component and for this reason they can worsen in a short period of time.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">4</span></a></p><p id="par0055" class="elsevierStylePara elsevierViewall">Liposarcoma usually presents as a heterogeneous and poorly defined mass. In magnetic resonance it shows as an iso-intense mass to muscle in T1, hyper-intense and contrasted T1 and in T2.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">5</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">Prognostic indicators are location, size at diagnostic and histological variant.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">4</span></a> Prognosis is relatively positive. Metastases are rare while recurrences are common.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">1</span></a></p><p id="par0065" class="elsevierStylePara elsevierViewall">Treatment is surgical excision of the tumor with adjuvant radio- and/or chemotherapy. According to Doyle, the most effective treatment is exenteration of the primary liposarcoma due to the high rate of relapses<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">8</span></a> and to its invasive capacity. Adjuvant radiotherapy has produced lower recurrence rates, although there is very little information about its use in liposarcoma.<a class="elsevierStyleCrossRef" href="#bib0110"><span class="elsevierStyleSup">10</span></a> The myxoid is the type that best responds to radiotherapy, while the well-differentiated type exhibits the worst response.<a class="elsevierStyleCrossRef" href="#bib0080"><span class="elsevierStyleSup">4</span></a></p><p id="par0070" class="elsevierStylePara elsevierViewall">The role of chemotherapy is controversial. The pleomorphic variant is highly resistant to this treatment.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">7</span></a> Mortality associated to systemic (not orbital) pleomorphic liposarcoma is of 40%.<a class="elsevierStyleCrossRef" href="#bib0075"><span class="elsevierStyleSup">3</span></a></p><p id="par0075" class="elsevierStylePara elsevierViewall">In the review of cases published by the authors between 2010 and 2015 (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>), 16 cases were found (including that of the authors) of primary orbital liposarcoma, 12 (75%) in males and 4 (25%) in females. The age ranged between 8 and 67 years, with an average of 44.75 years. It does not exhibit laterality preferences and is most frequently intraconal (62.5%). The most frequent presentation is in the form of proptosis. In the majority of cases, treatment was combined with one or more biopsies followed by exenteration in 11 cases. Four patients received radiotherapy and one received chemotherapy. Metastases are only referred in one case, while 2 deaths were associated to the tumor (12.5%).</p><p id="par0080" class="elsevierStylePara elsevierViewall">The most frequent histological variant was the well-defined type (6 cases, 37.5%), followed by the myxoid variant (3 cases, 18.75%), pleomorphic (3 cases, 18.75%). In 2 cases a combination of the myxoid and well-differentiated types was referred (12.5%), and one additional case did not specify variant (6.25%).</p><p id="par0085" class="elsevierStylePara elsevierViewall">Obital liposarcoma presentation is rare and of uncertain behavior. As liposarcoma is an infiltrating and recurring tumor, in addition to the associated mortality, broad resections (exenteration) are recommended in order to improve patient survival.
