(C, D) Linear regression.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Benign prostatic hyperplasia (BPH) is a urologic disorder common in males older than 50 years of age, responsible for symptoms at the lower urinary tract as many as 40% of them.<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a> Historically, transurethral resection of the prostate (TURP) was the surgical treatment of choice for prostates small and medium sized, open prostatectomy (OP) being reserved for large sized ones.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">2</span></a> The latter is an invasive procedure associated to a significant morbidity which requires a lower abdominal incision with long periods of hospitalization and recovery.</p><p id="par0010" class="elsevierStylePara elsevierViewall">Holmium laser enucleation of the prostate (HoLEP) is a relatively modern technique,<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">3</span></a> with the same indications as TURP or OP. The accumulated experience has showed that this technique is safe and effective, with clinical results that are comparable over time both with TURP<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">4,5</span></a> and with OP<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">6,7</span></a> and with a minimum risk of reoperation.<a class="elsevierStyleCrossRefs" href="#bib0200"><span class="elsevierStyleSup">8–10</span></a> From the anesthetic point of view, there are numerous advantages of HoLEP, since there is a lower morbidity having decreased the transfusion rates, the incidence of the TURP syndrome, etc.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a>. However, the published evidence is reduced almost entirely to studies aimed at urological aspects. The disadvantages, like the extended period of the intervention and the necessary learning curve, make us think about what is the ideal anesthetic technique and its relevant considerations.</p><p id="par0015" class="elsevierStylePara elsevierViewall">The aim of this study was the evaluation of the recent introduction of the (HoLEP) in our field through an analysis of the learning curve. Secondarily, the HoLEP was compared in 2 different phases (learning and stabilization) to the third retrospective group of control formed by the prostate retropubic adenomectomies (RA) based on terms of morbidity and anesthetic considerations.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Materials and methods</span><p id="par0020" class="elsevierStylePara elsevierViewall">A retrospective study was designed. After receiving approval from the Ethics Committee for Regional Clinical Research of the Principality of Asturias (n.° 128/13), the histories of patients undergoing surgery of BPH from March 2009 to February 2013 were revised by 2 medical specialists. The first 100 cases of HoLEP and the last 50 cases of retropubic adenomectomy were included. In the first ones, the learning curve was analyzed, from which 2 groups were established: HoLEP-A made of those patients that would make up the learning phase, and HoLEP-B formed by those patients in which it was considered that they had already reached the stabilization phase of the surgical technique. For this reason, the 100 patients were analyzed consecutively.</p><p id="par0025" class="elsevierStylePara elsevierViewall">All patients received antibiotic prophylaxis (1.5<span class="elsevierStyleHsp" style=""></span>g iv cefuroxime) and premedication. In the case of receiving anticoagulant or antiplatelet treatment, it was suspended and replaced according to the usual protocols following the recommendations marked by the Spanish Society of Anaesthesiology and Resuscitation.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">12</span></a> The HoLEP technique was performed by the same surgical team (2 expert urologists in endoscopic techniques), without the supervision of a mentor, and the technique outlined by Tan and Gilling<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">13</span></a> was followed including enucleation of prostatic lobes by holmium laser and subsequent morcellation of the fragments of prostate tissue. The equipment used was a 100<span class="elsevierStyleHsp" style=""></span>W holmium laser generator (VersaPulse PowerSuite™, Lumenis, Yokneam, Israel), a 550<span class="elsevierStyleHsp" style=""></span>μm laser fiber, a tissue morcellator (VersaCut™, Lumenis, Yokneam, Israel), endoscopic material (Karl Storz, Tuttlingen, Germany), a continuous irrigation of physiological saline, and a video system.</p><p id="par0030" class="elsevierStylePara elsevierViewall">Open adenomectomy was performed retropubically according to the technique described by Millin<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">14</span></a> by any member of the department of urology.</p><p id="par0035" class="elsevierStylePara elsevierViewall">Two types of study were performed; the first, following the methodology described in previous studies,<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">15,16</span></a> was used to establish the learning curve of our group and to be able to select the members of the HoLEP-A and HoLEP-B groups. For this, all patients were grouped into temporary consecutive subgroups, and the averages of the parameters to be studied were faced. They were analyzed using regression analysis and curve estimation on the cloud of points obtained, selecting equal or similar coefficients of determination (<span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span>), the explanatory equation of the simplest curve of all calculated. The parabolic inflection point of the curve was estimated as the way to establish the cut-off between HoLEP-A and HoLEP-B for the different subgroups.</p><p id="par0040" class="elsevierStylePara elsevierViewall">The second study was conducted comparing HoLEP-A, HoLEP-B, and RA groups, after establishing the first 2 with the previous study. The variables to be compared in the 3 indicated groups belong to all available data of hospitalization (data of the patient, of the intervention, of the anesthesia, perioperative variables) and of follow-up during the first 6 months (additional material) classifying the potential surgical complications of the first month through the Clavien–Dindo system.<a class="elsevierStyleCrossRefs" href="#bib0245"><span class="elsevierStyleSup">17–19</span></a></p><p id="par0045" class="elsevierStylePara elsevierViewall">The statistical analysis was performed with SPSS see. 22.0 (SPSS Inc., Chicago, U.S.A.). The qualitative variables are expressed as frequency and the quantitative variables as mean and standard deviation, even if they did not follow a normal distribution in order to facilitate understanding. The sample size conditioned a 95% potency for the analysis of qualitative variables. For the analysis of quantitative variables, an analysis of variance for an independent variable was used as long as the homogeneity of variances between groups was met. When that assumption was not met, a non-parametric analysis of variance (Kruskal–Wallis) was applied. Differences between groups were confirmed by post hoc tests (LSD in the first case and Games–Howell test in the second). In the evolution of the time of quantitative variables, a factorial analysis was performed within the general linear model for repeated samples, with T Pillai's statistic. In all analyses, a <span class="elsevierStyleItalic">p</span>-value lower than 0.05 was considered.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Learning curve of the prostate enucleation by holmium laser</span><p id="par0050" class="elsevierStylePara elsevierViewall">Patients were grouped into groups of 10 taken chronologically in a consecutive manner. Point clouds are shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>. The efficiency of enucleation also presented a linear equation 0.06<span class="elsevierStyleItalic">X</span><span class="elsevierStyleHsp" style=""></span>+<span class="elsevierStyleHsp" style=""></span>0.15<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span><span class="elsevierStyleItalic">y</span>, with <span class="elsevierStyleItalic">R</span><span class="elsevierStyleSup">2</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.87, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001 (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>D).</p><p id="par0060" class="elsevierStylePara elsevierViewall">Patients were also grouped into groups of 25 consecutive patients and their evolution was analyzed over time. There is a clear difference between group 1 and group 4. The first 25 operated patients have significantly higher intervention and enucleation times than the other groups. By contrast, the last 25 patients have significantly greater amount of resected prostate tissue, more efficient morcellation and enucleation, but also longer washing time and higher number of nights of stay. The rest of variables and groups did not reach statistical significance (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><p id="par0065" class="elsevierStylePara elsevierViewall">Therefore, it was estimated that the cut-off point between both groups was around the 50th position in the global group. Patients 0–50 formed the HoLEP-A group and patients 51–100 the HoLEP-B group.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Comparison between prostate enucleation groups using holmium-A laser, prostate enucleation using holmium-B laser and retropubic adenectomies</span><p id="par0070" class="elsevierStylePara elsevierViewall">150 patients were recorded in total, 50 cases per group. The groups were homogeneous among them regarding preoperative parameters such as age, weight, height, smoking, presence of antiplatelet and/or anticoagulant therapy, and ASA (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), except ultrasound prostate volume (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0075" class="elsevierStylePara elsevierViewall">Most anesthetic procedures were initially performed using spinal anesthesia (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>), although there was a percentage of reconversion in general anesthesia's: from 12.2% in HoLEP-A to 2.12% in HoLEP-B, compared to 8.33% of RAs, although without reaching statistical significance (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.19).</p><p id="par0080" class="elsevierStylePara elsevierViewall">Regarding intraoperative anesthetic complications, no significant differences were found. There was 6% of cases requiring intraoperative transfusion in the RA group vs. 0% of grouped HoLEPs (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05). Of the total 150 patients, only 3 needed postoperative mechanical ventilation and stay in resuscitation over 24<span class="elsevierStyleHsp" style=""></span>h. Two were RA and presented intraoperative bleeding hemorrhagic shock. The third, the HoLEP-A group, suffered a systemic inflammatory response syndrome (SIRS) by drilling of the prostate capsule (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>).</p><p id="par0085" class="elsevierStylePara elsevierViewall">The total operation time was lower in RA (126.80<span class="elsevierStyleHsp" style=""></span>min) vs. HoLEP-B (154.10<span class="elsevierStyleHsp" style=""></span>min) vs. HoLEP-A (192.30<span class="elsevierStyleHsp" style=""></span>min) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001), the difference being significant among the 3 groups (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01). Previous prostate volume was also higher in RA versus both HoLEPs (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001) being able to see the histogram thereof among the different groups and their evolution over time in <a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>. When comparing the two phases of the HoLEP and evaluating the efficiency of both phases, in group HoLEP-B we obtained greater efficiency of enucleation (grams of tissue enucleated per minute used) and morcellation (grams of morcellated tissue per minute used) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.001).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0090" class="elsevierStylePara elsevierViewall">There were no significant differences in the time of bladder catheterization or in the decrease of perioperative sodium. A difference was found in bladder irrigation time, being more than double in RA compared to HoLEPs (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01). Also in HoLEP-A less blood loss was recorded versus RA (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01), a finding not confirmed for HoLEP-B. Postoperative stay was significantly lower among HoLEP groups and RA group (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><p id="par0095" class="elsevierStylePara elsevierViewall">The highest percentage of intraoperative and medium-long term complications are described in the HoLEP-A group; however, according to the Clavien–Dindo system, in the first month increased incidence of surgical complications is observed in the RA group in the perioperative process (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>). According to hemorrhagic loss, transfusion requirements were significantly lower in the HoLEP groups (3% vs 18% in RA) (<a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a>), the same as the lowest amount of concentrates of transfused blood cells (2.33 vs. 4.67 in RA). No cases were reported in either group of perioperative mortality.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">Several procedures were associated in the same surgical procedure. In the HoLEP-A group, 2 patients were registered with bladder lithotripsy, and one with laparoscopic bladder diverticulectomy. In the HoLEP-B group, 2 bladder TURs, 2 bladder lithotripsies, one laparoscopic partial cystectomy, and one laparoscopic nephrectomy were performed. In the RA group, one patient needed a cystolithotripsy.</p><p id="par0105" class="elsevierStylePara elsevierViewall">Comparing the HoLEP technique over time (<a class="elsevierStyleCrossRef" href="#tbl0025">Table 5</a>), in urodynamic functional tests, no differences were found, although there was a statistically significant increase in Qmax more than doubled in HoLEP-A and tripled in HoLEP-B 3 months after surgery. Furthermore, PSA values over time did not differ between the 2 groups, but in both groups they had dropped almost to a third of the initial value (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0.01).</p><elsevierMultimedia ident="tbl0025"></elsevierMultimedia></span></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0110" class="elsevierStylePara elsevierViewall">In our study, the HoLEP technique demonstrates similar results to those found in previously published studies regarding benefits and disadvantages,<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">6,7</span></a> noting that once the learning phase (around 50 cases without mentor) is overcome, the incidence of complications is reduced, and better results are obtained in the different parameters assessed, regional anesthesia being the most widely used technique in an effective and safe manner.</p><p id="par0115" class="elsevierStylePara elsevierViewall">The occurrence of HoLEP has meant a change of perspective by simulating endoscopically open adenomectomy with which it is compared, being able to be performed on any prostate regardless of size without worsening the results obtained.<a class="elsevierStyleCrossRefs" href="#bib0260"><span class="elsevierStyleSup">20,21</span></a> Thus, HoLEP is the only laser technique for treating BPH presenting evidence level I according to the urological European guidelines.<a class="elsevierStyleCrossRefs" href="#bib0165"><span class="elsevierStyleSup">1,8</span></a></p><p id="par0120" class="elsevierStylePara elsevierViewall">The advantages of HoLEP are: minimizing surgical bleeding, reduction of transfusion requirements, eliminating the risk of dilutional hyponatremia, and the possible emergence of a TURP syndrome, reduction of bladder irrigation times, possibility of obtaining prostate tissue for histopathology, and similar results compared to all other alternatives in terms of improving symptoms and relief of bladder outlet obstruction in the medium to long term. All these benefits involve lower perioperative morbidity and shorter hospital stay.<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">4,6,7</span></a></p><p id="par0125" class="elsevierStylePara elsevierViewall">In our study, the overall transfusion rate was 3% in HoLEP versus 18% in RAs. In previous studies, an average of 7–14% transfusion is reflected for OPs,<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">1</span></a> being able to find according to the series highly variable values (0–24%).<a class="elsevierStyleCrossRef" href="#bib0270"><span class="elsevierStyleSup">22</span></a> The HoLEP technique has decreased these transfusion requirements with an overall incidence between 0% and 4%,<a class="elsevierStyleCrossRefs" href="#bib0180"><span class="elsevierStyleSup">4,22</span></a> noting that the increase in prostate size is not a cause of greater transfusion<a class="elsevierStyleCrossRef" href="#bib0275"><span class="elsevierStyleSup">23</span></a> and is defined as a safe technique in patients with anticoagulant medication.<a class="elsevierStyleCrossRef" href="#bib0280"><span class="elsevierStyleSup">24</span></a> In our study, 4% of the RAs had severe bleeding that required intensive care. There was a case of SIRS in the initial phase of the HoLEP. Urological sepsis cases are extremely rare in HoLEP, but they are described in the literature.<a class="elsevierStyleCrossRef" href="#bib0285"><span class="elsevierStyleSup">25</span></a> Lengthening of the operative time, described as one of the disadvantages of HoLEP, decreases with surgeon experience, and morcellation has also helped to reduce this time with respect to the first descriptions of this technique.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a> Another virtue of HoLEP, unlike other types of lasers, is that it allows, by not vaporizing tissues, us to postoperatively analyze the entire resected prostate tissue to confirm or rule out the presence of cancer. In our sample, 10% of incidental prostatic adenocarcinoma was detected in total HoLEPs which is within the values described (0–20%).<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">15</span></a></p><p id="par0130" class="elsevierStylePara elsevierViewall">The learning curve of HoLEP is another drawback, being considered long and complex by some authors.<a class="elsevierStyleCrossRefs" href="#bib0235"><span class="elsevierStyleSup">15,16</span></a> It is important to select cases at the beginning and take into account the potential impact that the introduction of a new technique involves until it is performed independently in suitable terms of time and morbidity.<a class="elsevierStyleCrossRef" href="#bib0290"><span class="elsevierStyleSup">26</span></a> Some authors have considered that the number of cases needed to overcome the learning curve is 50 cases in isolation, which would be reduced to 20 interventions under the supervision of an expert tutor<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">15</span></a> with whom an acceleration of the learning would be achieved. However, it is a controversial issue without a defined criterion, so that other authors estimate the cut-off point without supervision in 20–30 cases.<a class="elsevierStyleCrossRef" href="#bib0295"><span class="elsevierStyleSup">27</span></a> It has been observed that improvement of the learning curve has a direct effect on the decrease of complications,<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">16</span></a> and that the number of cases required to overcome this initial stage is similar to other urological procedures, such as robotic radical prostatectomy.<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">8</span></a> These ideas are consistent with what obtained in our series (with learning without external supervision) where we see that learning curves assume an inflection of around 50 patients (being able to consider it a suitable cut-off point) and a progressive increase in efficiency of the technique by accumulating experience.</p><p id="par0135" class="elsevierStylePara elsevierViewall">It seems clear that subarachnoid block is the anesthetic technique of choice commonly used in TURPs, by promoting rapid detection of a change in mental status,<a class="elsevierStyleCrossRefs" href="#bib0215"><span class="elsevierStyleSup">11,28</span></a> and in adenomectomies by presenting an analgesic effect in the early hours, avoiding abdominopelvic contractions and hypertensive peaks caused by pain which might be reasons for bleeding.<a class="elsevierStyleCrossRef" href="#bib0305"><span class="elsevierStyleSup">29</span></a> However, descriptions of which anesthetic techniques to be used in HoLEP do not exist, being reduced to the descriptive experience of the urologic literature. So, Placer et al. show a series of 125 cases where the performed anesthesia was general in 4%, spinal in 51.2%, or combined in 44.8%.<a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">16</span></a> However, Humphreys et al. reported that more than 90% received general anesthesia with laryngeal mask, while the rest received subarachnoid anesthesia.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">18</span></a> Other authors indicate that their procedures were performed using epidural or spinal anesthesia.</p><p id="par0140" class="elsevierStylePara elsevierViewall">The regional anesthetic technique was the most used. It is important to highlight the difference of rate of conversion to general anesthesia when spinal anesthesia was performed between the 2 HoLEP groups. The percentage of failure of the HoLEP-B group was 2.1%, which falls within the probability of failure of a spinal anesthesia that is established with an incidence of about 3.2%.<a class="elsevierStyleCrossRef" href="#bib0310"><span class="elsevierStyleSup">30</span></a> However, the high rate of reconversion (12.2%) in the learning phase of the HoLEP, before prostates with a lower average size and a lengthening of the total operating time, forces us to rethink whether a combined technique (spinal–epidural) could be the most appropriate at this phase. General anesthesia may be another added option in certain special cases, such as where there are deformities of the spine, high cardiac comorbidity, etc.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">11</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">Although the relevant data were not collected, there is a widespread perception of decreased analgesic requirements of HoLEP with regard to RAs. The presence of suprapubic pain is remarkable in 8% of the RAs after the first 2 months. Chronic pain can be caused by a surgical procedure,<a class="elsevierStyleCrossRefs" href="#bib0315"><span class="elsevierStyleSup">31,32</span></a> and the pain is often associated with significant morbidity with the consequent socioeconomic impact.<a class="elsevierStyleCrossRef" href="#bib0320"><span class="elsevierStyleSup">32</span></a></p><p id="par0150" class="elsevierStylePara elsevierViewall">A limitation of this study is the short time of follow-up (6 months) to define the long-term complications. Equally, the small sample size determines that some differences do not reach the expected significance or that spurious differences get it, as we think it happens in the dissimilarity between prostate volumes. These are different among the 3 groups, the statistical analysis indicating that they are higher in the RA group. However, the significant difference found among the 3 groups should be treated with caution, the temporary assessment of the techniques confirms how from the moment of entering the consolidation phase of the HoLEP technique, the RA practically has not been made in our field. In addition, and since then, some of the most voluminous prostates have been operated directly with HoLEP. Therefore, we believe that the differences found in this variable are not such because of the limited sample size of our series.</p><p id="par0155" class="elsevierStylePara elsevierViewall">Other limitations are the association of other surgical procedures that can act as confounding factor in certain variables, or data loss, especially in adenomectomies, being a retrospective study where certain variables such as urodynamic ones were not previously recorded. In any case, it is necessary to conduct further prospective studies with different anesthetic techniques to see if it is a factor that may influence morbidity as well as to clarify the safest and most effective anesthetic and surgical technique in large prostates.</p><p id="par0160" class="elsevierStylePara elsevierViewall">In conclusion, we can say that HoLEP is a technique that has been introduced and settled at our center as an alternative to open retropubic adenomectomies with good results in terms of morbidity and reduction of hospital stay. Regarding the learning curve, a cut-off point of around 50 patients can be considered, when performed without a mentor, as appropriate. We analyzed our learning curve for this technique, and we compared it in a secondary manner with prostatic adenomectomy.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Materials and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A retrospective comparative study was conducted that included the first 100 cases of HoLEP performed in our center and the latest 50 cases of retropubic adenomectomy. We collected data on the patients, the surgery, the anesthesia, the perioperative variables, the anesthesia complications and the postoperative variables, with a 6-month follow-up. We analyzed the learning curve without mentors for HoLEP and compared the characteristics of HoLEP in 2 separate phases (learning and stabilization phases) with the latest retropubic prostatic adenomectomies performed.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Intradural anesthesia was the most common technique. The transfusion needs, length of stay (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.01) and postoperative morbidity were lower for HoLEP than for adenomectomy. However, the retropubic adenomectomy group had larger initial prostate volumes (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001) and shorter surgical times (<span class="elsevierStyleItalic">P</span><span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>.001). Se recogen los datos del paciente, de la intervención, del acto anestésico, de las variables perioperatorias, de las complicaciones anestésicas y de las variables postoperatorias con un seguimiento durante 6 meses. Se analizó la curva de aprendizaje sin mentor del HoLEP y se compararon las características del HoLEP en 2 fases diferenciadas (fase de aprendizaje y fase de estabilización) con las últimas adenomectomías retropúbicas de próstata realizadas.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">La anestesia intradural fue la técnica más frecuente. Las necesidades de transfusión, el tiempo de estancia (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,01) y la morbilidad postoperatoria fueron menores en los HoLEP que en las adenomectomías. Sin embargo, el grupo de las adenomectomías retropúbicas tenía un mayor volumen prostático inicial (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001) y un menor tiempo quirúrgico (p<span class="elsevierStyleHsp" style=""></span><<span class="elsevierStyleHsp" style=""></span>0,001).
