Also included reasons at the various stages of this systematic review.</p>" ] ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Myringotomy with ventilation tube (VT) insertion is one the most commonly performed procedures in children and adolescent worldwide.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">1</span></a> According to recent guidelines, VT insertion is indicated with otitis media with effusion (OME) for at least 3 months or recurrent acute otitis media (AOM), at least three in the last 6 months or four or more episodes in the last 12 months.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">2</span></a> VTs usually remains in the eardrum between 6 and 12 months.<a class="elsevierStyleCrossRef" href="#bib0140"><span class="elsevierStyleSup">3</span></a> During this period otorrhea is the most frequent complications with an incidence of 30–83%. Upper respiratory tract infections (URTIs) are the most common risk factor associated with otorrhea in children with VT, but water penetrations have traditionally been implicated. “Can children with VTs be permitted to swim without any protection?” This is the question that otolaryngologists are often asked by the parents of their paediatric patients.<a class="elsevierStyleCrossRef" href="#bib0135"><span class="elsevierStyleSup">2</span></a> For years, parents have been advised to protect children's ears with VTs from contact with water, either by avoiding swimming or by protecting ears with ear plugs or antiwater caps, as water exposure in the middle ear such as during hair washing or bathing is likely to cause AOM. There is a growing evidence that water should not traverse VTs unless under significant pressure.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a> The most recent American Academy of Otolaryngology-Head and Neck Surgery Foundation (AAO-HNSF) clinical practice guidelines for VTs<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a> state that routine water precautions should not be prescribed in these patients. Despite these recommendations, many otolaryngologists and paediatricians continue to prescribe earplugs during bathing or swimming or to avoid aquatic activities. Currently in literature, there are already two reviews on this topic. There is a wide range Cochrane systematic review with strict selection criteria performed by Moualed et al.<a class="elsevierStyleCrossRef" href="#bib0155"><span class="elsevierStyleSup">6</span></a> suggesting that wearing earplugs reduces the rate of otorrhea in children with VTs; clinicians and parents should understand that the absolute reduction in the number of episodes of otorrhea appears to be very small and is unlikely to be clinically significant. Finally, in 2017 Steele et al.<a class="elsevierStyleCrossRef" href="#bib0160"><span class="elsevierStyleSup">7</span></a> conducted a review founding no compelling evidence of a need for water precautions. However, both these reviews presented some limitations. The first one used strict selection criteria and included only 2 high-quality studies. The second one presented evidence up to 2005 and it was a pooled review including not only the studies about this topic but also those concerning the prophylactic use of antibiotic eardrops after water exposure to reduce the otorrhea incidence.</p><p id="par0010" class="elsevierStylePara elsevierViewall">The goal of this review is to identify, summarize and critically appraise the current evidence concerning water precautions for children with VTs. To meet this goal, a comprehensive systematic literature review was performed, and the studies were analyzed by the authors.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Methods</span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">General study design</span><p id="par0015" class="elsevierStylePara elsevierViewall">The study was designed per the recommendations of the Centre for review and Dissemination's Guidance for Undertaking Review in Health Care<a class="elsevierStyleCrossRef" href="#bib0165"><span class="elsevierStyleSup">8</span></a> and is being reported in adherence with the Preferred Reporting Items for Systematic Review and Meta-Analyses (PRISMA) statement.<a class="elsevierStyleCrossRef" href="#bib0170"><span class="elsevierStyleSup">9</span></a></p></span></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Data source and study searching</span><p id="par0020" class="elsevierStylePara elsevierViewall">An electronic search was performed on PubMed/MEDLINE, Google Scholar, and Ovid databases. For the search strategy, combinations of the following search words were conducted MeSH: “No precautions”, “water precautions”, “swimming”, “non swimming”, “bathing”, “showering”, “hair rising”, “ear plugs”, “ear protective device”, “swimming caps”, “ear molds”, “ventilation tubes”, “grommets”, “tympanostomy tube”, “middle ear”, “otitis media with effusion”, “acute otitis media” and “otorrhea”. The other searches were adjusted to fit the specific requirements for each database. Then, a cross-reference search of the included studies was performed to minimize the risk of missing relevant data. The last search was run on April 2020.</p><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Inclusion/exclusion criteria</span><p id="par0025" class="elsevierStylePara elsevierViewall">The selection of studies was based on the Population, Intervention, Comparison and Outcome (PICO) framework.<a class="elsevierStyleCrossRef" href="#bib0175"><span class="elsevierStyleSup">10</span></a> The PICO process is a mnemonic used in Evidence-Based Medicine to frame and answer a clinical or health care related question. It presents the following structure.</p><p id="par0030" class="elsevierStylePara elsevierViewall">P (Population): paediatric population with middle ear VTs.</p><p id="par0035" class="elsevierStylePara elsevierViewall">I (Intervention): children submitted to VTs insertion with ear protection during water exposure; children submitted to VTs insertion without ear protection during water exposure.</p><p id="par0040" class="elsevierStylePara elsevierViewall">C (Comparison): children submitted to VTs insertion without ear protection during water exposure; children submitted to VTs insertion without water exposure.</p><p id="par0045" class="elsevierStylePara elsevierViewall">O (Outcome): otorrhea incidence after water exposure.</p><p id="par0050" class="elsevierStylePara elsevierViewall">S (Study design): prospective clinical comparative studies. Studies not in English, review, clinical guidelines, consensus statements, epidemiological studies, diagnostic studies, molecular/laboratory studies, observational studies, case studies, conference abstracts, letters, and studies with unclear and/or incomplete data were excluded.</p></span><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Data extraction and data analysis</span><p id="par0055" class="elsevierStylePara elsevierViewall">Two independent reviewers (A.M. and A.F.) separately searched for related scientific papers. All articles were initially screened by title and abstract, and then the authors independently assessed the full-text versions of each publication and excluded those whose content was judged not to be strictly related to the subject of this review. Data extraction from the included studies was systematically made using a structured form, and two reviewers (A.M., A.F.) independently checked it. A qualitative synthesis analysis was performed considering the selected studies regarding the effects of water exposition on paediatric subjected with VTs. Data of the studies included the population demographics and baseline characteristics, details on intervention and control conditions, study design, outcome, and time of measurement, as well as risk estimates. Discrepancies were identified and resolved through discussion.</p></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Study quality assessment</span><p id="par0060" class="elsevierStylePara elsevierViewall">The National Institute for Health and Clinical Excellence (NICE) quality assessment tool was used to evaluate the quality of the included studies.</p></span><span id="sec0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0100">Statistical analysis and summary of findings</span><p id="par0065" class="elsevierStylePara elsevierViewall">It was no possible to perform the intended statistical analysis and summary of findings as described in our protocol, due to heterogenic reporting style and lack of data in the individual studies included in this review. Thus, the effect on individual outcomes and overall quality assessment were solely narratively described. Only the data that was available in the retrospective studies was used. Authors of the included studies were not contacted for further information.</p></span></span><span id="sec0045" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0105">Results</span><p id="par0070" class="elsevierStylePara elsevierViewall">The flow diagram is shown in <a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a> (PRISMA Flow Diagram) depicts the selection process that includes 9 studies for 1299 patients. The baseline characteristics of the studies are reported in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. Four randomized clinical trial (RCT)<a class="elsevierStyleCrossRefs" href="#bib0130"><span class="elsevierStyleSup">1,4,11,12</span></a> and five prospective cohort studies<a class="elsevierStyleCrossRefs" href="#bib0190"><span class="elsevierStyleSup">13–17</span></a>were included. The number of subjects varied from 14<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a> to 399,<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a> the range age from 3 months<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">12</span></a> to 14 years<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">13</span></a> with a male prevalence in all the studies where gender was reported. The follow-up varied from 0.5 to 45 month.</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><span id="sec0050" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0110">Water exposure with and without ear protections (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>4)</span><p id="par0075" class="elsevierStylePara elsevierViewall">Subtil et al<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a>. investigated the otorrhea incidence for 6 months follow up period in 244 children submitted to VT insertion divided in two groups: 130 patients with ear protections during water exposure and 114 patients without any ear protections; no significant difference regarding the cumulative otorrhea incidence between the two groups was found (ear protection: 31.5% vs no protection: 21.9% <span class="elsevierStyleItalic">p</span> value: 0.092). Also Miyake et al.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">1</span></a> showed no significant difference about incidence of otorrhea between the ear protection group and no protection group (respectively 0.04<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.09<span class="elsevierStyleHsp" style=""></span>episodes/month vs 0.05<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.1<span class="elsevierStyleHsp" style=""></span>episodes/month; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.8). Moreover, Wang et al.<a class="elsevierStyleCrossRef" href="#bib0210"><span class="elsevierStyleSup">17</span></a> evaluated the tympanic membrane in 14 patients with VTs after a 1-hour water exposure without ear protection two weeks later. The authors collected the following results: eight ears dry, five ears with water over the outer 1 third of the external ear canal, two ears with water over the inner 2 thirds of the external ear canal, 6 ears with water on the tube or tympanic membrane and 1 ear with water penetration into the middle ear, supporting that middle ear infection are not likely when surface swimming.</p><p id="par0080" class="elsevierStylePara elsevierViewall">On the contrary, in 2005 Goldstein et al.<a class="elsevierStyleCrossRef" href="#bib0180"><span class="elsevierStyleSup">11</span></a> found that that there is a small but statistically significant increase in the otorrhea incidence in children who swim and bathe without ear protections than those with ear protections (protection group: 0.07<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.31<span class="elsevierStyleHsp" style=""></span>episodes/month vs no protection group: 0.10<span class="elsevierStyleHsp" style=""></span>±<span class="elsevierStyleHsp" style=""></span>0.31<span class="elsevierStyleHsp" style=""></span>episodes/month; <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.05).</p></span><span id="sec0055" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0115">Water exposure vs no water exposure (<span class="elsevierStyleItalic">n</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>2)</span><p id="par0085" class="elsevierStylePara elsevierViewall">Smelt and Yeoh<a class="elsevierStyleCrossRef" href="#bib0190"><span class="elsevierStyleSup">13</span></a> investigated the otorrhea incidence in 83 children with VTs divided in two groups: 43 swimmers without ear protections and 40 non-swimmers, showing no significant difference (7.5% vs 17.6%). Similarly, Silimy et al.<a class="elsevierStyleCrossRef" href="#bib0195"><span class="elsevierStyleSup">14</span></a> showed no statistical differences between 45 swimmers without ear protections and 41 non-swimmers (20% vs 34.1%).</p></span><span id="sec0060" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0120">Other studies</span><p id="par0090" class="elsevierStylePara elsevierViewall">Parker et al<a class="elsevierStyleCrossRef" href="#bib0185"><span class="elsevierStyleSup">12</span></a>. investigated 107 patients with VTs divided in three groups: group 1, 62 children swimmers without ear protection; group 2, 15 children swimmers with ear protections and group 3, 30 children non-swimmers. No significant difference was found in the otorrhea incidence between these three groups (respectively 67.7%, 86.7%, 60%, <span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.19).</p><p id="par0095" class="elsevierStylePara elsevierViewall">Becker et al.<a class="elsevierStyleCrossRef" href="#bib0205"><span class="elsevierStyleSup">16</span></a> evaluated 85 patients with VTs divided into three groups: group 1, 32 children swimmers without ear protections; group 2, 23 children swimmers with ear protections and group 3, 30 children no swimmers. The percentage of patients who developed at least one episode of AOM was 16% in the swimming without earplugs group and 30% in the swimming with earplugs group and in the no swimmers’ group, without any statistically significant differences.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Salata and Derkay<a class="elsevierStyleCrossRef" href="#bib0200"><span class="elsevierStyleSup">15</span></a> studied 399 paediatric patients with VTs divided in four groups: group 1, 138 children swimmers without ear protection; group 2, 101 children swimmers with antibiotic ear drops application after water exposure; group 3, 44 children swimmers with ear protection and group 4, 116 children non-swimmers. No statistically significant difference was observed in the incidence of post-tympanostomy swimming-related otorrhea among the 3 swimming groups (11%, 14%, and 20% of children in groups 1, 2, and 3, respectively, reported swimming-related otorrhea (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.26). Children who did not swim at all (group 4) did not differ significantly in their overall otorrhea incidence (59%) from the three swimming groups combined (68%) during the follow-up period (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.11).</p></span></span><span id="sec0065" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0125">Discussion</span><p id="par0105" class="elsevierStylePara elsevierViewall">In the common clinical practice, many otolaryngologists and paediatricians do not recommend exposing children with VTs to water. A recent survey on the most common precautions about water exposure after VT placement provided by UK clinicians showed that only of 10.4% of responders advised no water precautions.<a class="elsevierStyleCrossRef" href="#bib0215"><span class="elsevierStyleSup">18</span></a> However, based on the data emerging from our review, many randomized evidences suggest that these precautions are unnecessary. In particular, there is no statistically significant difference in otorrhea incidence between water exposure with and without ear protections in children with VTs. Moreover, it is not recommended the use of antibiotic eardrops as a prevention strategy to reduce the otorrhea incidence in children with VTs after water exposure. When VTs are inserted, it is traditionally assumed that chances of middle ear infections are increased by creating a passage for water into the middle ear: it is the rationale behind the advice to patients regarding precautions during water-based activities.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">19</span></a> However, although the rationale is sound, it has never been clearly demonstrated that it is a common occurrence in everyday clinical practice. Many practical exposure models showed that it is very unlikely for water to enter into the middle ear through VTs during swimming.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">20</span></a> There are two main explanations the water cannot pass through the VTs and therefore cannot enter the middle ear or the water pass through the VTs but does not cause infection. This is the real problem of water getting into the middle ear. Therefore, putting on ear protection is to prevent contaminated water from entering, not when clean water enters. Among the different pathogens, Pseudomonas aeruginosa is one of the most frequently isolated in pools and hot tubs which typically cause refractory otorrhea because it is capable of forming biofilms on the VTs.<a class="elsevierStyleCrossRef" href="#bib0230"><span class="elsevierStyleSup">21</span></a> The passage of water through VTs depends on several factors such as water pressure, surface tension, Eustachian Tube (ET) patency, different type VTs, water characteristics and time elapsed between the positioning of the VTs and the start of swimming. The pressure necessary for water to pass through a VT varies between 12.8 and 22.8<span class="elsevierStyleHsp" style=""></span>cm of water and even the soapy water the water pressure necessary was 10.95<span class="elsevierStyleHsp" style=""></span>cm of water.<a class="elsevierStyleCrossRef" href="#bib0235"><span class="elsevierStyleSup">22</span></a> It is known that soapy water reduces surface tension, thus lowering the pressure required to penetrate VTs.<a class="elsevierStyleCrossRef" href="#bib0225"><span class="elsevierStyleSup">20</span></a> The opening of the ET favours the passage of water through the VTs. However, when a child dives, the ambient pressure increases, and it is transmitted equally to both the external auditory canal and consequently to the tympanic membrane through the VT and to the nasopharynx through the nasal fossae. Consequently, there is an increase in nasopharyngeal pressure that impedes the passive opening of the auditory tube compensating the external ear canal pressure increase. This mechanism prevents the passage of fluids through the VTs.<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a> Brattmo et al. <a class="elsevierStyleCrossRef" href="#bib0240"><span class="elsevierStyleSup">23</span></a> observed that subjects with VTs had a poorer ability to equalize pressure gradients through the ET than those with healthy ears. These results support the theory that opening the Middle Ear (ME) to the atmosphere leads to a deterioration in its pressure regulation capacity as a consequence of gas diffusion over the ME mucosa. Regarding the size of VTs, it is known that larger tube may be associated with higher otorrhea incidence. Ungar et al.<a class="elsevierStyleCrossRef" href="#bib0245"><span class="elsevierStyleSup">24</span></a> used a printed 3D-model of the external auditory canal (EAC) and the ME to assess the permeability of various VTs to different fluids. The authors observed that water passed through the standard 1.14<span class="elsevierStyleHsp" style=""></span>mm diameter VTs after filling the EAC with a volume of 2<span class="elsevierStyleHsp" style=""></span>ml and through a narrower grommet or a T-tube after filling the canal with 2.5<span class="elsevierStyleHsp" style=""></span>ml. Soapy-water had the highest penetration in all VTs (1–2<span class="elsevierStyleHsp" style=""></span>ml). The initial volume of dexamethasone needed for penetration was 2.5<span class="elsevierStyleHsp" style=""></span>ml in the standard tubes. It did not pass at any volume through the narrow grommet or the T-tube. Moreover, Ricks et al.<a class="elsevierStyleCrossRef" href="#bib0250"><span class="elsevierStyleSup">25</span></a> compared the penetration ability of different water solutions (sea water, chlorinated water, freshwater and soapy water) for Shah, mini-Shah, T tube and titanium bobbin VTs thought a specific model in swimmers. For all VTs soapy water was the most penetrating, followed by seawater. Titanium bobbins required significantly less of each solution for penetration. Mini-Shah grommets required significantly more of all solutions except soapy water for penetration to occur. Shah grommets were more resistant to chlorinated and sea water than T tubes. Mini-Shah grommets appear to protect against water penetration into the middle ear cleft and their use should be considered in patients who are water-sport enthusiasts. Furthermore, swimmers in sea or chlorinated water seem to be at higher risk than freshwater swimmers. Titanium bobbins were relatively easily penetrated by all four solutions and should be avoided in keen swimmers. Mini-Shah grommets appear to protect against water penetration into the middle ear cleft and their use should be considered in patients who are watersport enthusiasts. Regarding the effects of different waters, it is advisable that precautions need to be taken while bathing in soapy water, swimming in chlorinated water or stagnant ponds and diving, but not when bathing in tap water, hair rinsing or surface swimming in the sea.<a class="elsevierStyleCrossRef" href="#bib0220"><span class="elsevierStyleSup">19</span></a></p><p id="par0110" class="elsevierStylePara elsevierViewall">Miyake et al.<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">1</span></a> presented the highest incidence and most patients with episodes of otorrhea, in the immediate post-operative period, of peripheral openings on the VTs, resulting from an ample myringotomy, which allows the inflow of water into the middle ear, causing infection. With the healing of the tympanic membrane, these openings should close, and the orifice of the VTs should remain as the only communication between the outer and middle ear, which would justify the decrease in late post-operative infection rate.</p><p id="par0115" class="elsevierStylePara elsevierViewall">Last but not least, ear protection, when prescribed, may significantly alter Quality of Life (QoL). It is understood that avoiding water is at least inconvenient and may delay learning to swim at worst.<a class="elsevierStyleCrossRef" href="#bib0150"><span class="elsevierStyleSup">5</span></a> Only one study<a class="elsevierStyleCrossRef" href="#bib0145"><span class="elsevierStyleSup">4</span></a> in our review, evaluated QoL, showing that the group with no protection improved from 84% (pre-surgery) to 87% (post-surgery) (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.004), and the group with protection improved from 84% to 87% (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.002) without any significant differences.</p><p id="par0120" class="elsevierStylePara elsevierViewall">The main limitations of this study have to do with our inclusion/exclusion criteria. First by limiting ourselves to English language only studies, we omitted current evidence concerning water precautions in the children with VTs. Second by excluding studies that compared the efficacy of antibiotic eardrops and spray to treat otorrhea after swimming as preventive strategy, we omitted other studies. However, it is our belief that these studies cannot be used to determine whether or not ear protection are required in the children with VTs. One of the most significant limitations of this review was the small number of articles and the few randomized clinical trials (only 4). Many studies suffered from a lack of randomization, blinding, and compliance checks and from small simple size. These few studies were characterized by a small patient population, short observation periods and different clinical and diagnostic parameters evaluated. In particular, differences were found regarding the different ways of diagnosing otorrhea incidence (assessed only by parents or confirmed by paediatrics or otolaryngology) and on the different follow-up period.</p><p id="par0125" class="elsevierStylePara elsevierViewall">The decision between protecting the ear or not when exposed to water should be individualized and discussed among the physician, the patient and family members. Protections should be recommended during the first month after surgery (protect against the risk of ample myringotomy<a class="elsevierStyleCrossRef" href="#bib0130"><span class="elsevierStyleSup">1</span></a>) and in cases of recurrent otorrhea. Patients suffering from recurrent-URTIs would particularly benefit from the use of ear protections.</p></span><span id="sec0070" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0130">Conclusion</span><p id="par0130" class="elsevierStylePara elsevierViewall">Based on the literature available, to allow water surface activities with no ear protection seems to present a minimum risk. To prohibit patients from swimming is an unnecessary procedure, and especially in children, hinders leisure times and the development of swimming and self-protection skills in water. In conclusion, we can make the following recommendations:<ul class="elsevierStyleList" id="lis0005"><li class="elsevierStyleListItem" id="lsti0005"><span class="elsevierStyleLabel">•</span><p id="par0135" class="elsevierStylePara elsevierViewall">Children can practice surface swimming without using ear protection. If ME infection and otorrhea do occur, it can be easy treat by ENT. A more careful evaluation is mandatory for children with VT suffering from recurrent URTIs, because the risk of ME infection is higher.</p></li><li class="elsevierStyleListItem" id="lsti0010"><span class="elsevierStyleLabel">•</span><p id="par0140" class="elsevierStylePara elsevierViewall">It is advisable to perform surface swimming activities in clean, chlorinated swimming pool preferably chlorinated. The risk of developing otorrhea increases by diving.</p></li><li class="elsevierStyleListItem" id="lsti0015"><span class="elsevierStyleLabel">•</span><p id="par0145" class="elsevierStylePara elsevierViewall">It is advisable to expose children with VTs to water no earlier than one month from the VTs placement in order to favour the healing of the tympanic membrane.</p></li><li class="elsevierStyleListItem" id="lsti0020"><span class="elsevierStyleLabel">•</span><p id="par0150" class="elsevierStylePara elsevierViewall">There are no comparative human clinical studies concerning other forms of water exposure such as hair washing, taking a shower, and bathing. However, according to the various experimental models available in the literature, it seems that it is not necessary to use ear protections during the shower and hair washing while it may be useful to use them when bathing in a bathtub with soap.</p></li><li class="elsevierStyleListItem" id="lsti0025"><span class="elsevierStyleLabel">•</span><p id="par0155" class="elsevierStylePara elsevierViewall">It is not recommended the use of antibiotic eardrops as a prevention strategy to reduce the otorrhea incidence in children with VTs after water exposure.</p></li></ul></p></span><span id="sec0075" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0135">Ethical approval</span><p id="par0160" class="elsevierStylePara elsevierViewall">No elsevierViewall">Myringotomy with ventilation tube (VT) insertion is one the most performed procedures in children and adolescents worldwide. VTs usually remain in the eardrum between 6 and 12 months and during this period otorrhoea is the most frequent complication. For years, parents have been advised to protect the ears of children with VTs from contact with water, as water exposure in the middle ear is likely to cause acute otitis media. However, there is a growing evidence that water should not traverse VTs unless under significant pressure, so routine water precautions should not be prescribed. Despite these recommendations, many otolaryngologists and paediatricians continue to prescribe earplugs during bathing or swimming or advise against aquatic activities. There are already two reviews in the current literature on this topic: the first used strict selection criteria and included only 2 high-quality studies, while the second presented evidence up to 2005. The aim of this review is to identify, summarize and critically appraise the current evidence concerning water precautions for children with VTs.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">Two independent reviewers separately searched for related scientific papers. A qualitative synthesis analysis was performed considering the selected studies regarding the effects of water exposure on paediatric subjects with VTs.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">Four randomized clinical trials (RCT) and five prospective cohort studies were included, for a total of 1299 patients aged from 3 months to 14 years. No statistically significant difference in otorrhoea incidence between water exposure with and without ear protection in children with VTs, and between water exposure and no water exposure in children with VTs, was found. Therefore avoiding water is at best inconvenient and at worst may delay learning to swim. The decision to protect the ear when exposed to water should be individualized and protection should be recommended during the first month after surgery and in cases of recurrent otorrhoea.</p></span> <span id="abst0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">Based on the literature available, allowing water surface activities with no ear protection seems to present a minimum risk, so it is not necessary to prohibit patients from swimming. Los TV normalmente permanecen sobre la membrana timpánica entre 6 y 12 meses, y durante este periodo una de las complicaciones más frecuentes es la presencia de otorrea. Durante muchos años, se les ha indicado a los padres la necesidad de evitar el contacto con agua de aquellos niños que tienen TV, ya que la exposición al agua puede producir otitis media aguda. Sin embargo, existe una creciente evidencia sobre la imposibilidad de que el agua pueda penetrar a través del TV a menos que exista una presión significativa y, por lo tanto, estas precauciones resultan innecesarias. A pesar de estas recomendaciones, muchos otorrinolaringólogos y pediatras siguen indicando tapones de oídos durante el baño o la natación e incluso la prohibición de deportes acuáticos. Actualmente en la literatura existen 2 revisiones sobre este tema: la primera utilizó criterios de selección estrictos e incluyó 2 estudios de alta calidad, mientras que la segunda presentó evidencia solo hasta el año 2005. El objetivo de esta revisión es identificar, resumir y evaluar críticamente la evidencia actual sobre las precauciones con el agua en los niños con TV.</p></span> <span id="abst0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0040">Métodos</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">Se realizó un análisis de síntesis cualitativa tomando en cuenta los estudios seleccionados sobre los efectos de la exposición al agua en niños con TV.</p></span> <span id="abst0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0045">Resultados</span><p id="spar0035" class="elsevierStyleSimplePara elsevierViewall">Se incluyeron 4 ensayos clínicos aleatorizados y 5 estudios de cohortes prospectivos para un total de 1.299 pacientes con edades comprendidas entre los 3 meses y los 14 años. No se encontró una diferencia estadísticamente significativa en la incidencia de otorrea entre la exposición al agua con y sin tapones para los oídos en niños con TV, ni entre la exposición al agua y la no exposición al agua en niños con TV. Se concluyó que evitar la exposición al agua representa una desventaja y, en el peor de los casos, puede retrasar el aprendizaje de la natación. La decisión entre proteger o no el oído en caso de exposición al agua debe ser individualizada, y se recomienda la protección durante el primer mes de postoperatorio y en casos de otorrea recurrente.</p></span> <span id="abst0040" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0050">Conclusión</span><p id="spar0040" class="elsevierStyleSimplePara elsevierViewall">Según la bibliografía disponible, permitir la realización de actividades acuáticas sin protección auditiva en pacientes con TV representa un riesgo mínimo, siendo innecesaria la prohibición de la natación.
