Abstract

Extracorporeal carbon dioxide removal (ECCO2R) is increasingly used in clinical practice as a means of facilitating protective mechanical ventilation and reducing the risk of ventilator-induced lung injury (VILI) in patients with acute respiratory distress syndrome (ARDS). In some cases, however, ECCO2R can worsen hypoxaemia due to various pathophysiological mechanisms that need to be taken into consideration. We report a case of refractory hypoxaemia that required interruption of ECCO2R therapy.

Resumen

La eliminación extracorpórea de dióxido de carbono (extracorporeal carbon dioxide removal, ECCO2R) es una técnica de creciente aplicación en la práctica clínica como estrategia adyuvante para garantizar una ventilación de protección pulmonar en pacientes con síndrome de distrés respiratorio agudo (SDRA), con el fin de disminuir la lesión pulmonar inducida por la ventilación mecánica (ventilator-induced lung injury, VILI). Sin embargo, en algunos casos, esta terapia puede asociarse a un empeoramiento de la hipoxemia debido a varios mecanismos fisiopatológicos que deben ser considerados por sus implicaciones clínicas. Presentamos un caso de hipoxemia refractaria durante terapia con un sistema de ECCO2R que obligó a su interrupción.
