Strategies for infection prevention in arthroplasties: Impact of the PREVINQ-CAT protocol

Abstract

Introduction

Surgical site infection (SSI) in arthroplasty is a common issue that increases morbidity, mortality, and hospital costs. This study evaluated the impact of the PREVINQ-CAT protocol on reducing SSI rates at Hospital de Mataró.

Methods

SSI data for total hip (THA) and knee arthroplasty (TKA) were collected from 2019 to 2023. The adherence and effectiveness of PREVINQ-CAT measures, implemented since 2022, were analyzed.

Results

The study included an annual sample of approximately 300 elective and urgent THA surgeries and 270 elective TKA surgeries. SSI incidence in THA peaked in 2021 (>4%) and decreased to 1.5% in 2023 following PREVINQ-CAT implementation, though without statistical significance (p = 0.47). In TKA, SSI remained stable at around 0.5%, with no significant differences between the 2019–2021 and 2022–2023 periods (p = 0.907). Adherence to appropriate antibiotic therapy increased from 70% to 90% in 2023 (p = 0.58). In 2022, only 0.3% of cases did not implement any measure, whereas in 2023, all interventions adhered to at least one, with two preventive measures being the most frequently applied (77.2% in 2022 and 78% in 2023).

Conclusion

The PREVINQ-CAT protocol contributed to reducing SSI in THA and maintaining low rates in TKA. Although the differences were not statistically significant, the high adherence to preventive measures highlights the importance of specific protocols in orthopaedic surgery.

La infección del sitio quirúrgico (ILQ) en artroplastias es un problema frecuente que aumenta la morbilidad, mortalidad y costes hospitalarios. Este estudio evaluó el impacto del protocolo PREVINQ-CAT en la reducción de las tasas de ILQ en el Hospital de Mataró.

Métodos

Se recopilaron datos de ILQ en prótesis total de cadera (PTC) y rodilla (PTR) entre 2019 y 2023. Se analizaron la adherencia y efectividad de las medidas del PREVINQ-CAT, implementadas desde 2022.

Resultados

El estudio incluyó una muestra anual aproximada de 300 cirugías electivas y urgentes de PTC y 270 electivas de PTR. La ILQ en PTC mostró un pico en 2021 (>4%), reduciéndose al 1,5% en 2023 tras la implementación del PREVINQ-CAT, aunque sin alcanzar significación estadística (p = 0,47). En PTR, la ILQ se mantuvo estable, con tasas en torno al 0,5%, sin diferencias significativas entre los periodos 2019-2021 y 2022-2023 (p = 0,907). La adherencia a la antibioterapia adecuada aumentó del 70% al 90% en 2023 (p = 0,58). En 2022, solo el 0,3% de los casos no implementó ninguna medida, mientras que en 2023 todas las intervenciones cumplieron al menos una, con una adherencia predominante de dos medidas preventivas (77,2% en 2022 y 78% en 2023).

Conclusión

El protocolo PREVINQ-CAT contribuyó a reducir la ILQ en PTC y a mantener bajas tasas en PTR. Aunque las diferencias no fueron estadísticamente significativas, la elevada adherencia a las medidas preventivas resalta la importancia de protocolos específicos en cirugía ortopédica.
