Abstract

Robot-assisted sparing mastectomy combined with immediate reconstruction using a prepectoral breast implant is a novel technique that provides good aesthetic results and high patient satisfaction. It was described for the first time by Antonio Toesca in 2015. In Spain, it is currently carried out in very few places. The results of this technique have been reported in multiple studies with low conversion and complication rates. It is indicated in patients with genetic risk and in patients with early breast cancer presenting no evidence of skin, chest wall or nipple-areolar complex involvement. In order to advocate the robotic approach, we give a step-by-step explanation so that it can be reproduced safely. However, we are aware that the technique entails a long learning curve and requires the availability of robotic equipment as well as experience in open breast cancer surgery.

Resumen

La mastectomía ahorradora asistida por robot combinada con reconstrucción inmediata con implante mamario prepectoral es una técnica novedosa con buenos resultados estéticos y alta satisfacción por parte de las pacientes. Fue descrita por primera vez por Antonio Toesca en 2015. En la actualidad en España se realiza en muy pocos sitios. Los resultados de esta técnica han sido comunicados en múltiples estudios en donde se evidencia baja tasa de conversión y baja tasa de complicaciones. Está indicada en pacientes con riesgo genético, y en aquellas pacientes con cáncer de mama temprano, sin evidencia de afectación de la piel, pared torácica o complejo areola-pezón. Con el objetivo de difundir el abordaje robótico, lo explicamos paso a paso para que pueda ser reproducido con seguridad. No obstante, somos conscientes que implica una amplia curva de aprendizaje, la disponibilidad del equipo robótico y experiencia en cirugía abierta en este tipo de cirugías, para poder alcanzarla.
