Laparoscopic repair of medial hernias (M1-2) can be performed safely and easily with self-gripping mesh on an outpatient basis. Its use provides atraumatic fixation in the entire anatomical structure, with less postoperative pain and sensation of a foreign body. This video illustrates the main steps of laparoscopic inguinal hernia repair (TAPP), focusing on how to deploy the self-gripping mesh while avoiding incorrect placement or folding. In addition, the vertical orientation of the mesh will ensure proper positioning over the Cooper ligament, thereby avoiding one of the main causes of recurrence. Pre-shaping of the mesh provides three-dimensionality for a better fit with the posterior inguinal wall (Fig. 1).
