Abstract

While pancreaticoduodenectomy is the standard for duodenal malignancy, pancreas-sparing duodenectomy (PSD) is a surgical option for benign duodenal disease. Robotic surgery for PSD offers advantages, such as enhanced visualization, improved instrument dexterity, and optimized dissection, while maintaining resection quality. After preoperative imaging review, we performed a totally robotic PSD with intracorporeal duodenojejunostomy in a 70-year-old patient with a 2.2-cm sessile polyp and dysplasia in the D3 portion of the duodenum. Although PSD is not indicated for malignancy, a negative intraoperative frozen section is critical before proceeding in select cases. A key learning point is combining robotic visualization with upper endoscopy to accurately define the distance between the ampulla and the lesion, ensuring oncologic margins. This manuscript outlines a stepwise surgical approach in a technically challenging scenario, aiming to preserve patient safety and provide an oncologically sound resection using totally robotic techniques.

Resumen

Aunque la pancreaticoduodenectomía es el procedimiento estándar para la malignidad duodenal, la duodenectomía con preservación pancreática (DPP) es una opción quirúrgica para la enfermedad benigna del duodeno. La cirugía robótica para DPP ofrece ventajas como una visualización mejorada, mayor destreza instrumental y una disección optimizada, manteniendo la calidad de la resección. Tras la revisión de las imágenes preoperatorias, realizamos una DPP totalmente robótica con duodenoyeyunostomía intracorpórea en un paciente de 70 años con un pólipo sésil de 2.2 cm con displasia en la porción D3 del duodeno. Aunque la DPP no está indicada para la malignidad, es fundamental contar con una biopsia por congelación intraoperatoria negativa antes de proceder en casos seleccionados. Un punto clave de aprendizaje es la combinación de la visualización robótica con la endoscopia alta para definir con precisión la distancia entre la ampolla y la lesión, asegurando márgenes oncológicos. Este manuscrito describe un abordaje quirúrgico paso a paso en un escenario técnicamente complejo, con el objetivo de preservar la seguridad del paciente y lograr una resección oncológicamente adecuada mediante técnicas totalmente robóticas.
