La Sociedad Española de Cirugía Torácica-Cardiovascular (SECTCV) tiene una serie de objetivos que se contemplan en el artículo V de sus estatutos. De entre ellos destaca: «Asegurar la calidad en el ejercicio de las especialidades correspondientes» 1. La calidad está referida a diversas propiedades de algo, tal y como define el Diccionario de la Real Academia Española2 en su vigésimo segunda edición, a la espera de posibles modificaciones en la vigésimo tercera. Una de las propiedades, referidas a nuestras actividades, es la del control de la actividad. Esto es un hecho conocido y fundamental, ya que siguiendo con esa definición de calidad 2, esa propiedad o conjunto de propiedades inherentes permiten juzgar el valor de lo que estemos analizando. Por lo tanto, el control y análisis de nuestra actividad profesional, individual o colectiva permite juzgar nuestro valor.

La SECTCV se ha distinguido, entre otros aspectos, por haber sido pionera en Europa en la recopilación y análisis de la actividad societaria en nuestro país a través del Registro Nacional de Intervenciones, publicado por primera vez en la Revista Española de Cardiología en 1988 3. A partir de 1995 ya se publicó en nuestra revista Cirugía Cardiovascular, de la mano de nuestros colegas y amigos R. Llorens, J. Silvestre, J.M.a Padró, J. Martinell, F. Villagrá y P.A. Sánchez, quienes contribuyeron al establecimiento y consolidación del registro 4. Con posterioridad y hasta la fecha, han contribuido en el mantenimiento del mismo J.M.a Cortina, J.M. Revuelta, E. Saura y A. Igual, habiendo sido estos dos en los últimos 15 años los que lo han dirigido 5. A todos ellos la SECTCV les está agradecida por el tremendo esfuerzo personal que, como otras actividades societarias, ha representado la actividad de recoger y analizar datos, además de darle la forma al registro para su publicación. Todos debemos felicitarnos porque la SECTCV pueda seguir ofreciendo una visión global y actualizada de la actividad de los cirujanos cardiovasculares en España. En Europa, tan sólo la Sociedad Alemana de Cirugía Torácica y Cardiovascular dispone de un registro de intervenciones societario más antiguo 6. Este ejemplo ha sido replicado por numerosas sociedades nacionales en el mismo intento de perseguir la calidad. El Registro de Intervenciones Valvulares del Reino Unido (UKHVR) inició la recolección de datos el 1 de enero de 1986 7, y es un ejemplo claro de persecución de la calidad en un área determinada.

Este número de Cirugía Cardiovascular incluye, de nuevo, el Registro Nacional de Intervenciones de la SECTCV 8 con los datos de 2006-2008. Diversas circunstancias han impedido la publicación del registro desde 2005 5, si bien en nuestra página de internet puede accederse a los datos brutos. Cualesquiera que hayan sido las causas, la no publicación del registro no deja de representar un error societario que ya hemos subsanado y que deseamos que no se repita. La confección de un registro nacional es una tarea ardua, que requiere gran esfuerzo personal y que no tiene gratificación mensurable en términos materiales para los que a ello, de forma voluntaria, se dedican. Sin embargo, el compromiso con los miembros de la SECTCV obliga a mantener unos estándares del más alto nivel. Los primeros responsables son los propios servicios que contribuyen al registro. Cuanto más tarde se remitan los datos a la sede de la SECTCV, más se retrasará la incorporación de los mismos a la base de datos general y su posterior análisis y la redacción del borrador. Los firmantes del registro, en nombre de la SECTCV, son, asimismo, responsables de que los datos lleguen refinados y correctamente presentados a un lector que desea conocer el estado de la especialidad en nuestro entorno. Por último, la junta directiva, como órgano directivo elegido por los miembros de la SECTCV, es asimismo responsable de que una información crítica, como la actividad global en el país, llegue al lector para que éste juzgue el nivel de calidad societario. Por lo tanto, hay diversos responsables a diversos niveles de actuación y cabe hacer notar que todos hemos sido responsables de este error, que sí es colectivo y que debería no repetirse en el futuro.

La voluntariedad del registro y la inclusión de datos de mortalidad lo hacen aún más valioso. Si bien los registros, como tales, tienen limitaciones desde el punto de vista científico, son herramientas admitidas cuyo valor radica en la descripción de la actividad diaria, de la vida real. El hecho es que cada vez tienen más importancia, y tenemos ejemplos claros de su valor. No hace mucho tiempo se publicó en estas páginas el Registro Alemán de la Disección de Aorta tipo A (GERAADA) 9, y se dejó entrever la idea de un Registro Europeo de las Enfermedades de la Aorta. Por último, la propia SECTCV tiene participación activa en el Registro de la European Association for Cardiothoracic Surgery (EACTS), tal y como puede consultarse en nuestra página de internet 1. Es decir, existe interés por la calidad.

Nuestro error multifactorial en cuanto a la publicación del Registro Nacional de Intervenciones no debe volver a ocurrir. Hay que mantener esta herramienta de calidad por encima de cualquier consideración, y lo ideal es que absolutamente todos los servicios contribuyeran con los datos de actividad y los de mortalidad. Hay que recapacitar y aprender de nuestros errores.

The Spanish Society of Thoracic-Cardiovascular Surgery (SECTCV) has several objectives that are contemplated in article V of its bylaws. Among them one can read: “To ensure the quality in its corresponding specialties” 1. Quality refers to the characteristics of something as defined by the Diccionario de la Real Academia Española2. One of the properties of our activities is the control of the activity performed. This is a well known and fundamental fact, because following the definition of quality 2 this characteristic allows to assess the value of what we are in the process of analyzing. Therefore, the control and analysis of our professional activity, individual or collective, gives us the chance to assess our value.

The SECTCV has been in Europe a pioneer in the collection and analysis of society activity through the National Registry of Interventions, which was first published in the Spanish Journal of Cardiology in 1988 3. From 1995 onwards it was always published in our journal Cirugía Cardiovascular by our colleagues and friends R. Llorens, J. Silvestre, J.M.a Padró, J. Martinell, F. Villagrá and P.A. Sánchez, who contributed to the consolidation of the registry 4. Thereafter and up to now J.M.a Cortina, J.M. Revuelta, E. Saura and A. Igual have contributed to maintain it, with latter two being responsible in the past fifteen years 5. The SECTCV is very grateful to all of them for the tremendous personal effort that, as in other society activities has represented the activity of data collection and analysis and the modeling of the registry for publication. We all have to congratulate ourselves because the SECTCV can continue to offer a global and updated view of the activity of cardiovascular surgeons in Spain. There is only one society in Europe, the German Society of Thoracic and Cardiovascular Surgery, having a Registry of Interventions older than ours 6. This example has been later replicated by a number of national societies in the same way of pursuing the quality. The United Kingdom Heart Valve Registry (UKHVR) started data collection by January 1, 1986 7 and is a very clear cut example of looking for quality in a given area.

This issue of Cirugía Cardiovascular incorporates again the National Registry of Interventions of the SECTCV 8 with data from 2006-2008. Several circumstances have precluded the publication of the registry from 2005 5 although through our website rough data can be accessed. Whichever the causes, the lack of publication of the registry is a society mistake that has been already fixed. We wish that this will not happen again in the future. Shaping a national registry is a tough task requiring a significant personal effort with no individual financial gain for those who volunteer to dedicate time to it. However, the commitment of the SECTCV makes compulsory to maintain the highest standards. The first responsible of the registry are the departments who contribute with their data. The later the data arrive in the headquarters of the SECTCV, the later will they incorporate into the database, the later the analysis will be performed and drafted. Those who, on behalf of the SECTCV, sign the registry are also responsible for the accuracy and refinement of the data and for the appropriate introduction to the reader who wishes to know the activity of the Specialty in our environment. Finally, Council of the SECTCV is also responsible for that this critical information, the overall activity in the country, reaches the reader for this to judge the quality of the society. Therefore, there are a number of society layers responsible at different levels and it has to be noticed that we all have been responsible for this is a collective mistake that should not be repeated in the future.

The registry is based on volunteering and the inclusion of mortality figures makes it even more valuable. Although registries, as such, have limitations from the scientific standpoint, there are well established tools whose value lies on the description of daily activity, of real life. They have more importance and we have some examples of their value. Very recently we have published the German Registry of Type A Acute Aortic Dissection (GERAADA) 9 and the eventual project of an European Registry of Diseases of the Aorta was also launched. Moreover, the SECTCV itself has an active participation in the Registry of the European Association foe Cardiothoracic Surgery (EACTS) as it can also be accessed through our website 1. In other words, there is interest on quality.

Our multifactorial mistake at the time of the publication of the National Registry of Interventions should never occur again. We have to keep this tool for quality assessment over any other consideration and ideal would be that absolutely all the Departments contribute to it with data of activity and mortality. We have to rethink and learn from our mistakes.
