Abstract

Congenital simple hamartoma of the retinal pigment epithelium is a rare, benign retinal lesion, rarely seen bilaterally. This case shows a 46-year-old man initially misdiagnosed with macular dystrophy, whose vision progressively declined over several years due to concomitant cataracts. Multimodal imaging, including OCT, was key in differentiating from more serious conditions such as choroidal melanoma. With no growth or secondary complications, the lesion remains under regular monitoring. This case underscores the importance of imaging in distinguishing CSHRPE from other chorioretinal disorders.

Resumen

El hamartoma congénito simple del epitelio pigmentario de la retina (HCSEPR) es una lesión benigna, poco frecuente, cuya presentación bilateral es excepcional. Presentamos el caso de un paciente de 46 años, inicialmente diagnosticado con distrofia macular, cuya agudeza visual fue disminuyendo progresivamente debido a cataratas concomitantes. La imagen multimodal, incluida la tomografía de coherencia óptica (OCT), fue fundamental para diferenciar estas lesiones de patologías más graves, como el melanoma coroideo. Ante la ausencia de crecimiento o complicaciones secundarias, se optó por un manejo conservador mediante seguimiento periódico. Este caso subraya la importancia de la imagen en el diagnóstico diferencial del HCSEPR frente a otros trastornos coriorretinianos.
