Abstract

[18F]FDG PET/TC is an emerging tool in the evaluation of inflammatory arthropathies, characterised by their insidious course and clinical overlap. It allows detection of subclinical inflammation, assessment of systemic involvement and quantification of metabolic parameters useful in early diagnosis and therapeutic monitoring. In rheumatoid arthritis, it correlates with clinical indices (Disease Activity Score), serological markers (CRP, anti-citrullinated protein antibodies) and ultrasound findings, and facilitates the identification of complications such as cardiovascular and pulmonary involvement. In spondyloarthritis, its ability to identify enthesitis and peripheral synovitis is promising, although less studied. In addition, [18F]FDG PET/TC evaluates the response to biological treatments, helping to optimise therapeutic decisions. However, its implementation faces challenges such as lack of standardised indications, incidental findings and technical artefacts. Although not routinely recommended, its usefulness in specific settings underlines the need for further studies to consolidate its role in the management of these diseases. In this manuscript, we present a brief review providing general and practical information about the role of [18F]FDG PET/TC in inflammatory osteoarticular pathology, while “Role and applications of [18F]FDG PET/CT in the assessment of osteoarticular infection and inflammation — Part I” has addressed its role in infectious osteoarticular disease.

La [18F]FDG PET/TC es una herramienta emergente en la evaluación de artropatías inflamatorias, caracterizadas por su curso insidioso y solapamiento clínico. Permite detectar inflamación subclínica, evaluar compromiso sistémico y cuantificar parámetros metabólicos útiles en el diagnóstico precoz y la monitorización terapéutica. En artritis reumatoide, se correlaciona con índices clínicos [puntuación DAS (Disease Activity Score)], marcadores serológicos (PCR, anticuerpos antiproteínas citrulinadas) y hallazgos ecográficos, y facilita la identificación de complicaciones como afectación cardiovascular y pulmonar. En espondiloartritis, su capacidad para identificar entesitis y sinovitis periféricas es prometedora, aunque menos estudiada. Además, la [18F]FDG PET/TC evalúa la respuesta a tratamientos biológicos, ayudando a optimizar decisiones terapéuticas. Sin embargo, su implementación enfrenta desafíos como la falta de indicaciones estandarizadas, hallazgos incidentales y artefactos técnicos. Aunque no está recomendado de rutina, su utilidad en escenarios específicos subraya la necesidad de más estudios para consolidar su papel en el manejo de estas enfermedades. En este manuscrito, presentamos una breve revisión proporcionando información general y práctica acerca del papel de la [18F]FDG PET/TC en la patología inflamación osteoarticular, mientras que en “Papel y aplicaciones de la [18F]FDG PET/TC en la evaluación de la infección e inflamación osteoarticular - Parte I” se ha abordado dicho papel en la osteoarticular infecciosa.
