Abstract

Anterior Cervical Discectomy and Fusion (ACDF) is a common surgical procedure with positive outcomes, but it is not without potentially serious complications for the patient.

We present the case of a patient who underwent ACDF and was found to have a cervical swelling prior to extubation. After a joint evaluation with the otolaryngology team, a suspicion of laryngeal structure dislocation was raised. A manual repositioning was performed prior to neuromuscular relaxation, and the patient was transferred intubated to the Recovery Unit due to a high risk of upper airway obstruction secondary to edema.

After 24 h of monitoring under scheduled corticosteroid treatment, a planned and safe extubation was carried out, witnessed by the otolaryngology team.

La discectomía y fusión cervical anterior (DFCA) es un procedimiento quirúrgico habitual con resultados positivos pero no carente de complicaciones potencialmente graves para el paciente.

Presentamos el caso de un paciente sometido a una DFCA, que previa a la extubación se evidencia una tumoración a nivel cervical. Tras la valoración conjunta con el equipo de otorrinolaringología se sospecha de una dislocación de las estructuras laríngeas. Se decide realizar una reposición manual previa a la relajación neuromuscular, y se traslada intubado a la Unidad de Reanimación por riesgo elevado de obstrucción de vía aérea superior secundaria a edema.

Tras 24 horas de vigilancia bajo tratamiento con corticoides pautados, se realiza una extubación planificada y segura presenciada por el equipo de otorrinolaringología.
