A 56-year-old woman presented at the Emergency Room with abdominal pain in the right iliac fossa. The physical examination revealed no signs of peritonitis, and no palpable mass at that level. Blood tests were unremarkable, with a slightly elevated CRP. The abdominal CT scan reported an uncomplicated right inguinal hernia containing the ipsilateral ovary.

In newborns, children and women of childbearing age, prompt surgery is mandatory to avoid loss of ovarian function. Due to the absence of alarm signs and the patient's age, surgery was scheduled 3 weeks later. A laparoscopic TAPP hernioplasty was performed, preserving the right adnexa. (video)
