The patient is a 67-year-old male who underwent colonoscopy due to a positive fecal occult blood test. Cystic lesions were evident 30 cm from the anal margin (Image 1). The biopsy was compatible with leiomyomatosis. An extension study was performed with a thoracoabdominal CT scan (Image 2).

It does not require surgical treatment, although patients may present symptoms of acute abdomen due to pneumoperitoneum.
