Abstract

A 66-year-old woman presents with unilateral orbital inflammation of several years of evolution, initially classified as idiopathic orbital inflammatory disease (IOID) subtype myositis. She presented with pain, eyelid inflammation and significant elevation of intraocular pressure, as well as radiological signs of exophthalmos and superior rectus myositis. She was treated with corticosteroids, with partial response. She was diagnosed with colon adenocarcinoma, complicated with febrile neutropenia, and therefore immunosuppressive treatment was contraindicated. Treatment with intraorbital rituximab was decided, achieving resolution of symptoms and reduction of IOP. The diagnosis of neoplasia raises the possibility of paraneoplastic orbital inflammation.

Resumen

Mujer de 66 años con inflamación orbitaria unilateral de varios años de evolución, inicialmente clasificada como enfermedad inflamatoria orbitaria idiopática (EIOI) subtipo miositis. Presentaba dolor, inflamación palpebral y elevación importante de la presión intraocular, así como signos radiológicos de exoftalmos y miositis de recto superior. Fue tratada con corticoides, con respuesta parcial. Fue diagnosticada de adenocarcinoma de colon, complicada con neutropenia febril, por lo que se contraindica la inmunosupresión. Se decidió el tratamiento con rituximab intraorbitario, logrando la resolución de los síntomas y reducción de la PIO. El diagnóstico de neoplasia plantea la posibilidad de que se tratase de una inflamación orbitaria paraneoplásica.
