The effect of hemostatic sutures on open suprapubic prostatectomy outcomes: A retrospective observational study

Abstract

Objective

To evaluate the effect of suturing the bladderneck in benign prostatic hyperplasia surgery on bleeding parameter sand surgical outcomes.

Methods

The age, comorbidities, preoperative findings, surgical treatment techniques, peroperative and postoperative bleedingrates, blood transfusion rates, complication rates, surgery note sand postoperative patient follow-ups of the patients operated on for benign prostatic hyperplasia were taken from the patient files. Patients who were sutured to the bladder neck and those who were not were divided into two group sand compared in terms of bleeding and surgical results.

Results

A total of 170 patients were included in the study, 106 patients whounder went bladder neck suturing and 64 patients who did not undergo bladder neck suturing. While the mean operation time in hemostatic suture applied group 84.2 ± 7.8 min, this time was 61.4 ± 6.3 min hemostatic suture not applied group (p < 0.001). The mean decrease in hemoglobin levels at the 2nd postoperative hour in the suturing group was observed to be greater than in the non-suturing group (2.1 g/dl, 2 g/dl, respectively). There was no statistical difference in postoperative blood transfusion rates (15.1%, 15.6%, respectively, p = 0.137). Postoperative complications and follow-updata were similar for bothgroup. Bladder neck contracture was higher rate (7.5% vs. 3.1%) in hemostatic suture applied group but ıt was not statistically significant.

Conclusion

Application of hemostatic suture prolongs the operation time of open prostatectomy without affecting bleeding parameter sand postoperative results. For this reason, open prostatectomy surgery is a surgical method that can be performed safely without applying sutures.

Objetivo

Evaluar el efecto de la sutura del cuello vesical en la cirugía de la hiperplasia benigna de próstata (HBP) sobre los parámetros de sangrado y los resultados quirúrgicos.

Métodos

La edad, las comorbilidades, los hallazgos preoperatorios, las técnicas quirúrgicas, las tasas de hemorragia perioperatoria y postoperatoria, las tasas de transfusión, las tasas de complicaciones, la nota quirúrgica y el seguimiento postoperatorio de los pacientes operados de hiperplasia benigna de próstata se extrajeron del historial de los pacientes. Los pacientes se dividieron en dos grupos según la colocación de sutura del cuello vesical y se compararon en términos de hemorragia y resultados quirúrgicos.

Resultados

En total, 170 pacientes fueron incluidos en el estudio, de los cuales 106 fueron tratados con sutura del cuello vesical y 64 no. El tiempo quirúrgico medio en el grupo en el que se aplicó sutura hemostática fue de 84,2 ± 7,8 minutos, y en el grupo en el que no se realizó sutura hemostática fue de 61,4 ± 6,3 minutos (p < 0,001; p < 0,001). Se observó que la disminución media de los niveles de hemoglobina en la 2ª hora postoperatoria en el grupo de sutura era mayor que en el grupo sin sutura (2,1 g/dl,2 g/dl, respectivamente). No hubo diferencias estadísticamente significativas en las tasas de transfusión postoperatoria (15,1%, 15,6%, respectivamente, p = 0,137). Las complicaciones postoperatorias y los datos de seguimiento fueron similares en ambos grupos. La contractura del cuello vesical fue mayor (7,5% frente a 3,1%) en el grupo en que se colocaron suturas hemostáticas, pero no fue estadísticamente significativa.

Conclusión

El uso de suturas hemostáticas prolonga el tiempo quirúrgico de la prostatectomía abierta sin afectar a los parámetros de hemorragia ni a los resultados postoperatorios. Por este motivo, la prostatectomía abierta se puede realizar de forma segura sin la colocación de suturas.
