Abstract

The purpose of this investigation was to test the influence of the observer's experience on the diagnostic capacity of different levels of misfit on a 3-unit implant supported screwed metal restoration, by analysing digital x-rays with different vertical angulations. A screwed metal restoration, supported by 3 implants, was fabricated and mounted on an acrylic model under controlled conditions. Between the structure and one of its supporting implants, 3 different situations of prosthetic fit/misfit were experimentally created (0μm, 200μm, 330μm), one of them considered as correct and the other two as incorrect. Each situation was then radiographed under standard conditions, with 5 different vertical angulations (0, 5, 10, 15 and 20 degrees). The 15 digital radiographies obtained were analysed by 25 clinicians (minimum of 4 years experience in implant prosthodontics) and 25 under-graduate students with no experience in implants prosthodontics. Images observation was performed under specific conditions and were classified as “correct passive fit” or “incorrect passive fit”. Chi-square test and descriptive statistics were performed. Results showed no statistically significant difference between the 2 groups of observers, regardless of radiographic angulation or prosthetic marginal gap. Distribution of correct diagnostic answers between the 2 groups was very similar or even identical. A high prevalence of “false-positive” answers was found in both groups. In conclusion, the results of the present study showed that observer's experience did not influence the diagnostic capacity of prosthetic misfit in an implant-supported screwed metal restoration through the analysis of digital x-rays with different vertical angulations.

O objectivo deste trabalho de investigação foi testar a influência da experiência do observador na capacidade de diagnóstico de estruturas metálicas implanto-suportadas com diferentes graus de adaptação através da análise de radiografias digitais executadas com diferentes angulações verticais. Foi fabricada uma estrutura metálica implanto-suportada por 3 implantes, montados em condições controladas num modelo acrílico. Entre a estrutura metálica e os implantes foram criadas experimentalmente 3 condições diferentes de adaptação protética (0μm, 200μm e 330μm), sendo uma considerada correcta e duas incorrectas. Cada situação foi radiografada em condições padronizadas, com diferentes angulações verticais (0, 5, 10, 15 e 20 graus). Obtiveram-se assim 15 radiografias digitais diferentes. As radiografias foram classificadas por 25 Médicos Dentistas com um mínimo de 4 anos de experiência em reabilitação fixa implanto-suportada e 25 alunos pré-graduados, sem experiência em reabilitação implanto-suportada. A observação das imagens foi sujeita a regras e a classificação constava de “Adaptação passiva da estrutura correcta” ou “Adaptação passiva da estrutura incorrecta”. Foi utilizado o teste de χ2 e estatística descritiva. Os resultados mostraram não haver diferença estatisticamente significativa entre os 2 grupos de observadores, independentemente da desadaptação protética ou da angulação radiográfica. A distribuição de respostas diagnósticas correctas pelos 2 grupos foi semelhante. Destaca-se o elevado número de “falsos-positivos” encontradas nos 2 grupos. Em conclusão, de acordo com o presente estudo, a experiência do observador não influenciou a capacidade de diagnóstico de estruturas metálicas implanto-suportadas com diferentes graus de desadaptação, mediante a análise de radiografias digitais executadas com diferentes angulações.
