Abstract

Emphysematous osteomyelitis is an extremely rare entity consisting of the presence of intraosseous gas that can extend to the joints and adjacent soft tissues. It is an aggressive infectious process associated with high mortality, especially in patients with risk factors such as tumors or diabetes mellitus. Because early diagnosis and immediate treatment are crucial to prevent the potentially devastating consequences of this condition, imaging tests such as computed tomography play a fundamental role in its diagnosis and management. Therefore, radiologists must be aware that intraosseous gas is a rare but alarming sign that is pathognomonic of emphysematous osteomyelitis, especially in the axial skeleton.

La osteomielitis enfisematosa es una entidad extremadamente rara consistente en la presencia de gas intraóseo, con posible extensión a las articulaciones y tejidos blandos adyacentes. Constituye un cuadro infeccioso agresivo y con alta mortalidad, especialmente en pacientes con factores de riesgo como neoplasias o diabetes mellitus, en el que las pruebas de imagen, tales como la tomografía computarizada (TC), desempeñan un papel fundamental. El diagnóstico precoz y un tratamiento inmediato son cruciales para prevenir las consecuencias potencialmente devastadoras de la enfermedad. Por lo tanto, el radiólogo debe ser consciente de que el gas intraóseo es un signo raro pero alarmante, patognomónico de la enfermedad especialmente en el esqueleto axial.
