We present a resource technique for the treatment of incisional hernia in patients who are transplant recipients of abdominal organs. The patient is a 56-year-old patient with a history of pulmonary hypertension and liver transplantation, who later developed a giant incisional hernia and portal thrombosis. The patient presented many symptoms associated with the incisional hernia, but surgical repair with mesh had been ruled out as it would increase abdominal pressure. The postoperative recovery was satisfactory.
