A 68-year-old man with a pacemaker and mechanical aortic valve, with a history of aparoscopic cholecystectomy (Fig. 1).

Computed tomography was performed due to abdominal discomfort, weight loss and elevated carcinoembryonic antigen, and peritoneal nodules were observed in the right flank suggestive of peritoneal carcinomatosis.

Colonoscopy and gastroscopy showed no suspicious lesions.

The study was not completed with magnetic resonance imaging because he had a pacemaker and a mechanical heart valve, and surgical exploration of the abdominal cavity was indicated.

During surgery, 13 intra-abdominal gallstones were found to be completely peritonised in relation to the radiological findings, with no evidence of carcinomatosis or other malignant lesions.
