The patient is a 67-year-old male who underwent colonoscopy due to a positive fecal occult blood test. Cystic lesions were evident 30 cm from the anal margin (Image 1). The biopsy was compatible with leiomyomatosis. An extension study was performed with a thoracoabdominal CT scan (Image 2).
After excision, analysis of the surgical specimen identified intestinal cystic pneumatosis.
Intestinal cystic pneumatosis is a rare benign entity of unknown etiology characterized by submucosal cystic formations. The presentation is incidental, and patients are usually asymptomatic. It does not require surgical treatment, although patients may present symptoms of acute abdomen due to pneumoperitoneum.
