We present a video about a surgical technique using the falciform ligament for vascular reconstruction in a case of adenocarcinoma of the head of the pancreas with portal vein involvement, requiring partial resection to obtain negative resection margins. This is an easy-to-obtain graft, with no need to sacrifice autologous veins; it involves a low risk of infection, low cost, and no required long-term anticoagulant treatment. This technique is a good resource in pancreatic surgery, especially in cases in which vascular involvement is not diagnosed preoperatively (ciresp.2019; 97: 54–61) (Figs. 1 and 2).
