A 92-year-old patient was evaluated in the emergency room due to intense diffuse abdominal pain that had progressed over the previous 6h, with signs of peritoneal irritation on examination. An urgent computed tomography scan showed an image of dilated small bowel loops that appeared to be located concentrically around the vessels in a radial pattern (“wagon wheel sign”) in the right iliac fossa (Fig. 1). These findings were consistent with intestinal volvulus and probable associated vascular compromise. Urgent surgery revealed an internal hernia due to an adhesion involving a 180cm segment of the jejunum-ileum, which required resection.
