Diagnosis of ocular toxocariasis by detecting antibodies in the vitreous humor

Abstract

Objectives

To demonstrate the production of the local toxocara antibodies in the vitreous of patients with negative serology.

Material and methods

Retrospective review of the medical records of patients seen in a Tertiary Referral Hospital, and at an Eye Institute, with an ocular diagnosis of toxocariasis who had a negative serology and positive titers in the vitreous.

Results

After reviewing the medical records of 40 patients with a diagnosis of ocular toxocariasis, a total of six cases (15%) were found with a negative serology and a positive vitreous titer for toxocara antibody, both of them performed using an ELISA procedure. The mean age was 18 years, there were no differences observed between males and females. A peripheral granuloma was the most common clinic finding, and all patients underwent vitrectomy either to remove epiretinal membranes or to repair retinal detachments.

Conclusion

A negative serology does not rule out a diagnosis of ocular toxocariasis. Vitreous samples may be necessary to confirm a clinical diagnosis.

Objetivos

Demostrar la producción de anticuerpos locales en el humor vítreo de pacientes con toxocariasis ocular con serologías negativas.

Material y métodos

Se realizó un análisis retrospectivo de las historias clínicas de pacientes con toxocariasis ocular examinados en un hospital general y en una clínica especializada que presentaron serologías negativas por ELISA para Toxocara canis (IgG) y ELISA positivo para Toxocara en muestras de humor vítreo.

Resultados

Se revisaron 40 historias clínicas de pacientes con diagnóstico clínico de toxocariasis ocular. En 6 de ellas (15%) se hallaron resultados negativos en las pruebas serológicas con valores positivos de anticuerpos en el humor vítreo. La edad media fue de 18 años y la relación entre sexos fue de 1:1. La presentación más frecuente fue el granuloma periférico, y a todos los pacientes se les realizó vitrectomía con el objetivo de extraer membranas prerretinales o reparar desprendimientos de retina.

Conclusión

La serología negativa no descarta el diagnóstico de toxocariasis ocular. Muestras del humor vítreo pueden confirmar la presencia de anticuerpos contra el parásito.
