"apellidos" => "Ozdemir" ] ] "afiliaciones" => array:1 [ 0 => array:2 [ "entidad" => "Health Sciences University, Gaziosmanpaşa Education and Research Hospital, Urology Clinic, Estambul, Turkey" "identificador" => "aff0005" ] ] "correspondencia" => array:1 [ 0 => array:3 [ "identificador" => "cor0005" "etiqueta" => "⁎" "correspondencia" => "Corresponding author." ] ] ] ] "titulosAlternativos" => array:1 [ "es" => array:1 [ "titulo" => "Factores de riesgo independientes asociados al sangrado en la nefrolitotomía percutánea: estudio retrospectivo" ] ] "textoCompleto" => "<span class="elsevierStyleSections"><span id="sec0005" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0065">Introduction</span><p id="par0005" class="elsevierStylePara elsevierViewall">Percutaneous nephrolithotomy (PNL) is commonly used for treatment of large or complex stones within the kidney since the development of the technology.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> Currently, guidelines recommend it as the first choice for all kidney stones >20<span class="elsevierStyleHsp" style=""></span>mm, and as a choice for stones 10−20<span class="elsevierStyleHsp" style=""></span>mm in size or lower pole stones <10<span class="elsevierStyleHsp" style=""></span>mm in size.<a class="elsevierStyleCrossRefs" href="#bib0010"><span class="elsevierStyleSup">2,3</span></a> In spite of developments in technology and prevalence of application, 23.3% rates of complications are observed with PNL.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,5</span></a> When complications associated with PNL were assessed in a systematic review of nearly 12,000 patients, 10.8% fever, 7% transfusion, 1.5% thoracic complications, 0.5% sepsis, 0.4% organ injury, 0.4% embolization, 0.2% urinoma and 0.05% mortality were observed.<a class="elsevierStyleCrossRef" href="#bib0020"><span class="elsevierStyleSup">4</span></a> The most common and concerning complication during and after the PNL procedure is hemorrhage. Some factors predicting hemorrhage during the PNL procedure were determined; these include body mass index (BMI), diabetes mellitus, stone size, hardness, degree of hydronephrosis, dilatation method, access numbers and operation time.<a class="elsevierStyleCrossRefs" href="#bib0030"><span class="elsevierStyleSup">6–8</span></a> Though factors related to disease were determined for hemorrhage related to PNL, insufficient studies were performed related to independent risk factors. We aimed to study the effect of other factors, like atherosclerotic vein disease causing endothelin function disorder with an important role in blood clotting especially.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Patients and methods</span><p id="par0010" class="elsevierStylePara elsevierViewall">Retrospective data from a total of 308 adult patients (211 men, 97 women) who underwent the PCNL procedure from 2013 to 2020, with no anatomic anomaly, previous kidney operation history on the same side, urinary infection and tendency toward hemorrhage were included in the study. All patients were assessed for preoperative non-contrast spiral tomography, renal functions, hemostasis and urinary infection as recommended.<a class="elsevierStyleCrossRef" href="#bib0045"><span class="elsevierStyleSup">9</span></a> To calculate renal stone Hounsfield unit (HU) values, the PACS Dicom Viewer-V2.7 software was used for digital imaging and assessment. For determination of mean HU values, 1<span class="elsevierStyleHsp" style=""></span>cm diameter circular areas were used for calculations. The presence of atheroma plaque was based on the appearance of atheroma plaque in veins within the non-contrast, thin section computed tomography (NCCT) image area and atherosclerotic vein disease was accepted. Obesity was assessed by measuring the fat tissue on the anterior abdominal wall with NCCT. Kidney anatomy and stone size were evaluated using NCCT. Stone dimensions were calculated according to European Association of Urology guidelines.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a></p><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Surgical technique</span><p id="par0015" class="elsevierStylePara elsevierViewall">All patients had open-ended 6 Fr urethral catheter inserted with cystoscopy in the lithotomy position under general anesthesia and were then placed in prone position. Opaque matter was administered through the urethral catheter to observe the pelvic calyceal system and an 18<span class="elsevierStyleHsp" style=""></span>G needle was inserted in the target calyx accompanied by C-arm fluoroscopy. Then a guidewire was inserted into the collecting system and the tract was dilated with Amplatz dilator or high-pressure balloon dilator to 30 Fr and a 30 Fr Amplatz sheath was inserted. Nephroscopy using 26<span class="elsevierStyleHsp" style=""></span>F optics fragmented the stones with pneumatic ultrasonic and/or laser lithotripters and removed the fragments with forceps. At the end of the procedure, nephrostomy catheter was inserted for drainage after the majority of procedures.</p><p id="par0020" class="elsevierStylePara elsevierViewall">All patients had antibiotic (cefazolin) prophylaxis administered. The difference between preoperative hemoglobin (Hgb) values and postoperative 1st day hemoglobin values was recorded. One unit blood transfusion increased hemoglobin (HB) level by 1<span class="elsevierStyleHsp" style=""></span>g/dl and hematocrit (Htc) value by 3%. For this reason, the falls in HB and Htc values were calculated using this formula ([preoperative Hb (Hct)<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>postoperative Hb (Hct)]<span class="elsevierStyleHsp" style=""></span>−<span class="elsevierStyleHsp" style=""></span>[number of units transfused<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>1g<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>dL (3% for Hct) Hb per unit transfused]).<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> This variation was assessed for the effect of gender, age, atherosclerotic vein disease, preoperative urine pH and density, leukocyte count, lymphocyte count, neutrophil count (NEU), platelet count, mean platelet volume (MPV), neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), stone volume, HU and obesity.</p></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Statistical method</span><p id="par0025" class="elsevierStylePara elsevierViewall">Descriptive statistics are given as mean, standard deviation, minimum and maximum values for continuous data. For statistical comparison of data, continuous data were analyzed with the Kolmogorov-Smirnov analysis for fit to normal distribution and the <span class="elsevierStyleItalic">t</span>-test was used for parametric data. To investigate the common variation in data, the Spearman correlation analysis was used. For interpretation of correlation coefficients, 0.0–2.4 was weak, 0.25−0.49 was moderate, 0.5−0.74 was strong and 0.75–1.00 was accepted as very strong. For statistical significance, p values below 0.05 in the 95% confidence interval were accepted as significant.</p></span></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Results</span><p id="par0030" class="elsevierStylePara elsevierViewall">In our study, Hgb variation was minimum −2.4, maximum 8.3 and mean 2.1 (standard deviation 1.6) (<a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>). There were positive, significant and weak correlations between Hgb variation with NEU (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.019), MPV (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.000), NLR (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.005), stone volume (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.041), and HU (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.024) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). There was a negative, significant and weak correlation between Hgb variation and PLT values (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.022) (<a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a>). There was no effect identified for presence of atheroma plaque (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.949) and gender (P<span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.078) (<a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>).</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><span id="sec0030" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0090">Discussion</span><p id="par0035" class="elsevierStylePara elsevierViewall">In PNL, among treatment methods chosen for renal stone treatment, the most common and concerning complication during and after the procedure is hemorrhage.<a class="elsevierStyleCrossRef" href="#bib0005"><span class="elsevierStyleSup">1</span></a> This situation is associated with morbidity and mortality and is the most important factor affecting treatment success. To reduce risks of complications like this, the AUA recommend non-contrast CT screening and full blood count including platelets.<a class="elsevierStyleCrossRef" href="#bib0015"><span class="elsevierStyleSup">3</span></a> All patients included in our study underwent these recommended procedures, as well as examination of creatinine for renal functions and urine culture to exclude urinary infection.</p><p id="par0040" class="elsevierStylePara elsevierViewall">Just as there are studies stating that the balloon dilatation method is more reliable for tract dilatation method, there are publications stating that neither is superior.1 In our study, all PNL procedures had the balloon dilatation method applied.</p><p id="par0045" class="elsevierStylePara elsevierViewall">During PNL, negligible hemorrhage permitting the procedure to continue is reported with observation in irrigation fluids, in addition to serious hemorrhage requiring transfusion at certain rates in studies. A study by Turna et al. including data for 193 patients found staghorn stones, multiple tracts, presence of diabetes and large stones were associated with increased renal hemorrhage during PCNL.<a class="elsevierStyleCrossRef" href="#bib0030"><span class="elsevierStyleSup">6</span></a> Another study including 370 patients identified that staghorn stones, high BMI, large stones, prolonged operation time and absence of hydronephrosis were significantly associated with risk of severe bleeding during PCNL.<a class="elsevierStyleCrossRef" href="#bib0035"><span class="elsevierStyleSup">7</span></a> A study including 649 patients found multiple access tracts, staghorn calculi, presence of diabetes and prolonged operation time, but not surgical experience, significantly increased blood loss during PCNL.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> A review from 2006 reported transfusion rates were 11.2–17.5% for patients undergoing the PNL procedure.<a class="elsevierStyleCrossRef" href="#bib0060"><span class="elsevierStyleSup">12</span></a> Recent large-series studies reported similar transfusion rates, apart from anomalous situations.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,13</span></a> We identified the transfusion rate was 10.2% for the data included in our study.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Studies assessing total hemorrhage, both negligible and requiring transfusion, reported mean fall in Hgb was 2.1–3.3<span class="elsevierStyleHsp" style=""></span>g/dl.<a class="elsevierStyleCrossRef" href="#bib0040"><span class="elsevierStyleSup">8</span></a> For the data in our study, we identified the mean fall in Hgb as 2.1<span class="elsevierStyleHsp" style=""></span>g/dl. Though previous studies were from periods when laser technology was not commonly used for stone fragmentation and our study results are from a period when laser technology is used more often, the similar outcomes lead to consideration that this may be an important observational finding.</p><p id="par0055" class="elsevierStylePara elsevierViewall">In hemorrhage control, von Willebrand factor (vWF) is released when vein endothelial integrity is disrupted and causes platelet adhesion and then platelet aggregation. This situation is called primary hemostasis. The platelets with activated clotting factors are wrapped in a fibrin network and thrombus occurs and adheres to the defective area. This process is called secondary hemostasis. A normal hemostasis system is important for hemorrhage control and vein endothelium acts as clotting factor for primary hemostasis, while platelets act as clotting factor for secondary hemostasis.<a class="elsevierStyleCrossRefs" href="#bib0070"><span class="elsevierStyleSup">14–16</span></a></p><p id="par0060" class="elsevierStylePara elsevierViewall">For vascular hemostasis, the main regulator is vein endothelium and atherosclerosis accepted as a systemic disease that causes endothelial dysfunction.<a class="elsevierStyleCrossRef" href="#bib0085"><span class="elsevierStyleSup">17</span></a> In our study, we did not identify a statistical difference for Hgb delta between groups with and without calcified atherosclerosis identified.</p><p id="par0065" class="elsevierStylePara elsevierViewall">Though the mechanism is not fully explained, aging is known to cause endothelial function disorder along with other endothelial disorders.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> In our study, we did not identify any effect of aging or gender on hemoglobin variation.</p><p id="par0070" class="elsevierStylePara elsevierViewall">Platelets are included in primary hemostasis with normal count 150−450<span class="elsevierStyleHsp" style=""></span>×<span class="elsevierStyleHsp" style=""></span>10<span class="elsevierStyleSup">9</span> and volumes 1.5−3<span class="elsevierStyleHsp" style=""></span>μm.<a class="elsevierStyleCrossRef" href="#bib0095"><span class="elsevierStyleSup">19</span></a> Platelet counts and volumes were within this interval for patient data included in our study. Though platelet count has a reducing effect on hemorrhage amounts, we identified the increase in mean platelet volume had an increasing effect on hemorrhage amount.</p><p id="par0075" class="elsevierStylePara elsevierViewall">NLR is a marker associated with inflammation and was shown to be independently associated with negative clinical outcomes of symptomatic intracranial hemorrhage, cancer and cardiovascular disease and various clinical situations.<a class="elsevierStyleCrossRef" href="#bib0100"><span class="elsevierStyleSup">20</span></a> In our study, we did not identify an effect on hemorrhage in PNL.</p><p id="par0080" class="elsevierStylePara elsevierViewall">PLR was determined to be an informative marker revealing deviations in platelet and lymphocyte counts due to acute inflammatory and prothrombotic situations. PLR, combined blood cell counts and ratios were investigated comprehensively in predictable immune suppression and neoplastic disease accompanied by thrombosis. Several large observational studies used values for deviated PLR in assessment of the severity of systemic inflammation and for prediction of infections and other comorbidities in inflammatory rheumatic diseases.<a class="elsevierStyleCrossRef" href="#bib0105"><span class="elsevierStyleSup">21</span></a> In our study, we did not identify an effect on hemorrhage in patients undergoing PNL.</p><p id="par0085" class="elsevierStylePara elsevierViewall">During the procedure and until healing, urine forming in the renal pelvis is in contact with the created tract line. Urine pH and density vary from patient to patient.</p><p id="par0090" class="elsevierStylePara elsevierViewall">Experimental studies showed a relationship between the lack of increased fibrinogen production in acidosis, accelerated consumption and disrupted coagulation.<a class="elsevierStyleCrossRefs" href="#bib0110"><span class="elsevierStyleSup">22,23</span></a> The clot formation time increased 168% when the pH fell from 7.4 to 6.8.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a></p><p id="par0095" class="elsevierStylePara elsevierViewall">Previous studies did not research the effect of urine pH on hemorrhage in patients undergoing PNL. In our study, we did not identify any effect of urine pH and density on hemorrhage in our PNL patients.</p><p id="par0100" class="elsevierStylePara elsevierViewall">Limitations of the study comprise the retrospective nature and limited number of patients. As this is the first study of this type, the results were not compared. It should be supported by studies in which a large number of prospective patients will be included.</p></span></span><span id="sec0035" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0095">Conclusions</span><p id="par0105" class="elsevierStylePara elsevierViewall">In addition to HU and stone volume, NEU count, MPV, NLR and PLT count may be used as independent risk factors for prediction of hemorrhage in PNL. Disease-related conditions (such as stone size, stone HU, tract number, and diameter) affecting this situation were determined. To determine independent risk factors that may affect the amount of hemorrhage in PNL.</p></span> <span id="abst0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0015">Material and method</span><p id="spar0025" class="elsevierStyleSimplePara elsevierViewall">A total of 308 adult patients (211 men, 97 women) undergoing the PNL procedure were included in the study. Renal anatomy and stone size were evaluated using non-contrast thin-section computed tomography (NCCT). NCCT was used to assess Hounsfield unit (HU) values of kidney stones, presence of atheroma plaque and obesity. The difference between preoperative hemoglobin (Hgb) values and postoperative 1st day Hgb values was recorded. This variation was evaluated for the effect of gender, age, atherosclerotic vein disease, urine pH and density, leukocyte count, lymphocyte count, neutrophil count (NEU), platelet count, mean platelet volume (MPV), neutrophil lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR), stone volume, HU, and obesity.</p></span> <span id="abst0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0030" class="elsevierStyleSimplePara elsevierViewall">The mean Hgb variation was identified as 2.1 (standard deviation: 1.6).
