Abstract

Post traumatic headache is a common condition that can be managed with pharmacologic interventions or analgesic procedures; however, most evidence is derived from patients with mild trauma, leaving a large gap with regard to patients with moderate or severe trauma who present complex pain. Botulinum toxin plays an increasingly important role in pain management. This neurotoxin acts on different receptors, ranging from TRPV1 (transient receptor potential vanilloid type 1) to CGRP (calcitonin gene-related peptide). This is the first case report of the use of perineural botulinum toxin in a patient with moderate post-traumatic headache who responded poorly to standard interventional measures.

La cefalea postraumática es una afección frecuente que puede tratarse con intervenciones farmacológicas o procedimientos analgésicos; sin embargo, la mayoría de las pruebas se basan en el paciente con traumatismo leve, lo que deja un gran vacío en los pacientes con traumatismo moderado o grave que presentan un dolor complejo. La toxina botulínica tiene un papel cada vez más importante en el tratamiento del dolor. Se ha descrito que tiene efectos sobre diferentes receptores que van desde el TRPV1 (receptor transitorio potencial vanilloide tipo 1) hasta la acción sobre el CGRP (péptido relacionado con el gen de la calcitonina). Este es el primer reporte de un caso de uso de toxina botulínica perineural en una cefalea postraumática moderada con baja respuesta a las medidas intervencionistas habituales.
