Abstract
Background

Pain is a common experience among mechanically ventilated patients. Pain among mechanically ventilated patients is aggravated by factors such as stage of illness, invasive procedures, and surgical interventions.

Objectives

To examine the pain behavior of patients on mechanical ventilation during nursing interventions.

Methods

A descriptive cross-sectional study. Data were collected from 150 mechanically ventilated patients at government hospitals at Sana’a -Yemen. Pain was evaluated before and during the nursing interventions via the "Ramsay Sedation Scale" and "Behavioral Pain Scale". The difference between pain level and hemodynamic parameters before and during the procedure was assessed using the Wilcoxon signed-rank test. A p value of 0.05 was considered to indicate statistical significance.

Results

The interventions that resulted in differences in pain scores were bed bathing, tracheal suction, oropharyngeal suction, nasogastric intubation, oral care, repositioning, perineum care, and intravenous cateterización through fluctuations all physiological parameters during these procedures.

Conclusion

Patients on mechanical ventilation feel pain before and during nursing intervention. In fact, harmless and comforting operations can injure patients. Nurses practice must take care-related suffering linked with their interventions into consideration while caring for critically sick nonverbal patients. It might be useful to use changes in vital signs as the main indicator of pain.

Antecedentes

El dolor es una experiencia común entre los pacientes con ventilación mecánica. El dolor entre los pacientes con ventilación mecánica se agrava por factores como la etapa de la enfermedad, los procedimientos invasivos y las intervenciones quirúrgicas.

Objetivos

Examinar el comportamiento del dolor de los pacientes con ventilación mecánica durante las intervenciones de enfermería.

Métodos

Estudio transversal descriptivo. Se recopilaron datos de 150 pacientes con ventilación mecánica en hospitales gubernamentales de la ciudad de Sana'a, Yemen. Se evaluó el dolor antes y durante las intervenciones de enfermería mediante la "Escala de sedación de Ramsay" y la "Escala de dolor conductual". La diferencia entre el nivel de dolor y los parámetros hemodinámicos antes y durante el procedimiento se evaluó mediante la prueba de rangos con signo de Wilcoxon. Se consideró que un valor p de 0,05 indicaba significación estadística.

Resultados

Las intervenciones que derivaron en diferencias en las puntuaciones de dolor fueron el aseo en cama, la succión traqueal, la succión orofaríngea, la intubación nasogástrica, el cuidado oral, el reposicionamiento, el cuidado del perineo y la cateterización intravenosa a través de fluctuaciones de todos los parámetros fisiológicos durante estos procedimientos.

Conclusión

Los pacientes con ventilación mecánica sienten dolor antes y durante la intervención de enfermería. De hecho, las operaciones inofensivas y reconfortantes pueden lesionar a los pacientes. La práctica enfermera debe tener en cuenta el sufrimiento relacionado con el cuidado vinculado a sus intervenciones al atender a pacientes no verbales gravemente enfermos. Podría ser útil utilizar los cambios de los signos vitales como el principal indicador de dolor.
