Perforation of the eyeball during retrobulbar anesthesia is rarely reported. A case of inadvertent ocular perforation during retrobulbar anesthesia in cataract surgery is presented. Retinal complications and follow up after six months are described.
A 58-year-old man presented with vision loss following cataract surgery on his right eye by phacoemulsification and intraocular lens implantation under retrobulbar anaesthesia performed one week earlier. Visual acuity was 0.05. Fundus examination revealed a vitreous haemorrhage in the lower area. A rounded haemorrhage was present next to the lower temporal arcade with a possible entry orifice. Optical coherence tomography, tomographic angiography and fluorescein angiography showed retinal ischaemia in the juxtapapillary macular area.
The patient was followed for 6 months. Visual acuity improved to 0.5. Optical coherence tomography and tomographic angiography showed a reduction of the edema in the retinal layers, with a persistent loss of the inner nuclear and outer plexiform layer.
Accidental intravitreal injection of bupivacaine and lidocaine during retrobulbar anaesthesia can result in permanent visual impairment. Extreme attention should be paid to the injection technique to avoid this complication.
La perforación del globo ocular durante la anestesia retrobulbar es poco frecuente. Presentamos un caso de perforación ocular accidental durante la anestesia retrobulbar en cirugía de cataratas. Se describen las complicaciones retinianas y el seguimiento a los seis meses.
Un varón de 58 años presentó pérdida de visión tras una cirugía de cataratas del ojo derecho mediante facoemulsificación e implante de lente intraocular bajo anestesia retrobulbar, realizada una semana antes. La agudeza visual era de 0,05. El examen del fondo de ojo reveló un hemovítreo en la zona inferior. Destacaba la presencia de una hemorragia redondeada adyacente al arco temporal inferior con un posible orificio de entrada. La tomografía de coherencia óptica, la angiografía tomográfica y la angiografía fluoresceínica mostraron isquemia retiniana en el área macular yuxtapapilar.
La paciente fue seguida durante 6 meses. La agudeza visual mejoró a 0,5. La tomografía de coherencia óptica y la angiografía tomográfica mostraron una reducción del edema en las capas retinianas, con pérdida persistente de la capa nuclear interna y la capa plexiforme externa.
La inyección intravítrea accidental de bupivacaína y lidocaína durante la anestesia retrobulbar puede provocar alteraciones visuales permanentes. Debe extremarse el cuidado con la técnica de inyección para evitar esta complicación.
