Introducción

El síndrome postrombótico de miembros inferiores es una secuela crónica y frecuente tras una trombosis venosa profunda. La clínica varía desde edema, dolor y claudicación venosa, en las fases iniciales, hasta ulceración y discapacidad, en las tardías. El tratamiento ha evolucionado en los últimos años desde la terapia compresiva y la cirugía de derivación venosa hasta la recanalización endovascular.

Caso clínico

Mujer de 40 años, con dolor y claudicación venosa incapacitante de la extremidad inferior izquierda, secundaria a trombosis venosa profunda iliofemoral, por lesión de la vena ilíaca primitiva izquierda durante cirugía de fijación intervertebral. En la exploración había edema importante de la extremidad con pulsos presentes. La ecografía recogía, en el lado izquierdo, incompetencia del sistema venoso profundo y obstrucción de la vena ilíaca común. Con anestesia general se practicó una flebografía intraoperatoria, vía vena femoral común izquierda. Se recanalizó la obstrucción y se liberaron dos stents autoexpandibles: el proximal, ligeramente introducido en la vena cava, y el distal, extendido en la ilíaca externa. No hubo complicaciones peri-operatorias y fue dada de alta al día siguiente con anticoagulación. A las seis semanas había disminuido notablemente el edema y había desaparecido la claudicación. La ecografía de control mostraba permeabilidad de los stents.

Conclusiones

Las técnicas endovasculares ofrecen actualmente resultados excelentes en el tratamiento del síndrome postrombótico, con una resolución más fácil y menos arriesgada que la cirugía y con la ventaja de una indicación más temprana. Sin embargo, requieren seguimiento y controles ambulatorios por medio de imagen.

Introduction

Post-thrombotic syndrome of the lower limbs is a frequent chronic sequela following deep vein thrombosis. The clinical features range from oedema, pain and vein claudication, in the early phases, to ulceration and disability in the later stages. In recent years treatment has advanced from compressive therapy and vein bypass surgery to endovascular recanalisation.

Case report

We report the case of a 40-year-old female with pain and disabling vein claudication in the lower left limb, secondary to iliofemoral deep vein thrombosis, caused by injury to the left primitive iliac vein during intervertebral fixation surgery. The examination revealed an important oedema in the limb with pulses present. Ultrasonography showed incompetence of the deep vein system and obstruction of the common iliac vein, on the left-hand side. Intraoperative phlebography was performed via the left common femoral vein under general anaesthesia. The obstruction was recanalised and two self-expanding stents were deployed: the proximal one was introduced slightly into the vena cava and the distal one was expanded within the external iliac. No perioperative complications occurred and the patient was discharged from hospital the next day with anticoagulant treatment. At six weeks the oedema was markedly reduced and claudication had disappeared. The control ultrasound study showed the stents were patent.

Conclusions

Endovascular techniques currently offer excellent results in the treatment of post-thrombotic syndrome and are performed more easily and with lower risk than surgery. They also have the advantage of being indicated at an earlier stage. Nevertheless, they require outpatient follow-ups and controls by means of imaging.
