Resumen
Objetivo

Revisar nuestros resultados en el tratamiento de reestenosis carotídeas post-endarterectomía tratados mediante técnicas endovasculares.

Material y métodos

Ocho pacientes afectos de estenosis carotídea localizadas en la zona previamente intervenida quirúrgicamente. Solamente uno de los pacientes presentó sintomatología en la reestenosis; excepto en este caso, el descubrimiento de las reestenosis fue mediante seguimiento de la endarterectomía primaria con eco-doppler color.

La técnica empleada fue la angioplastia transluminal percutánea (ATP), con colocación posterior de un stent si aquella no había sido satisfactoria: una luz arterial lisa de calibre regular y no inferior al de la carótida interna distal. En los casos en los que se colocó un stent se valoró la correcta aposición de este a la pared arterial realizando un Eco-Doppler color intraoperatorio.

Resultados

El procedimiento tuvo éxito en todos los casos. únicamente en un paciente fue posible solucionar el problema con una dilatación aislada, siete precisaron de la colocación de un stent. La permeabilidad a largo plazo es del 100% en los ocho pacientes tratados.

Conclusiones

Creemos que esta técnica puede ser útil en grupos seleccionados de pacientes, ya que permite obtener. una buena permeabilidad a largo plazo con una morbimortalidad baja. No obstante son necesarios estudios prospectivos y randomizados comparándolo con la cirugía abierta para definir sus indicaciones correctas.

Summary
Objective

To review our results in the endovascular treatment of carotid reestenosis.

Material and methods

Eight patients presented with carotid stenotic lesions in a previously endarterectomiced segment. Only one patient had a symptomatic reestenosis while seven were diagnosed by color Duplex follow up of the primay carotid endarterectomy.

Carotid Percutaneous Transluminal Angioplasty (PTA) was the standard technique used, stents were used only when PTA had a suboptimal result; a lumen surface other than a smooth, regular surface of a diameter not smaller than the distal internal carotid artery.

When stents were used, the correct wall apposition of all stents was assesed by intraoperative Color Duplex.

Results

The eight patients were considered technical success. Seven patients were stented while only one stenosis resolved with just a PTA.

Long term permeability is 100%.

Conclusions

We believe carotid endovascular surgery to be an usefull technique to treat this selected group of patients with a low morbidity and mortality and a high long term permeability. But we also believe that a prospective randomiced study comparing open and endovascular carotid surgery is needed to further define each technique indications.
