Abstract

In recent years, the concept of mechanical power has gained new interest in the intensive care community when it was recognised that ventilator parameters can interact with forces acting on the lung surface and contribute to ventilator-induced lung injury. This represents the unwanted result of a complex interaction between various mechanical forces, which act on the lung structures during mechanical ventilation. The degree of direct damage may depend on the amount of energy transferred from the mechanical ventilator to the patient's lungs. The transfer of power depends on the ventilatory parameters adjusted by the operator at the bedside. Mechanical power represents mechanical energy multiplied by the respiratory rate, and reflects the amount of energy applied to the respiratory system per minute during mechanical ventilation. Mechanical power is a summary variable, including all potential causes of ventilator-induced lung injury: tidal volume, driving pressure, flow, respiratory rate, and positive pressure at the end of expiration. To represent the meaning of mechanical power, the classical equation of motion of the respiratory system is required as the sum of the resistive flow pressures and the elastic pressures, multiplied by the tidal volume and the respiratory rate, achieving an expression of total energy delivered to the respiratory system in one minute.
