Abstract
Introduction

Prothrombin complex concentrates are now one of the major drugs to prevent bleeding complications in patients anticoagulated with warfarin. It has 4 advantages for correcting the coagulation compared to fresh frozen plasma: 1) Lower risk of disease transmission, 2) Faster and complete vitamin K-dependent coagulation correction factors, 3) A lower volume infused, avoiding complications associated with blood transfusion; and 4) Avoid risks immunological incompatibility groups.

Materials and methods

An observational, descriptive, retrospective study of patients admitted to the emergency department of the Hospital Universitario San Ignacio (level IV) during the period December 2010 to May 2014 with active bleeding secondary to ingestion of warfarin with an INR>2, and treated with prothrombin complex concentrate. The analysis included 49 patients with a mean age of 69 years, and 51% were women. The most common complication of anticoagulation in this patient group was 32% intracranial haemorrhage, and 28.6% digestive tract bleeding. In all patients the INR was brought back to normal after the administration of the prothrombin complex concentrate. More than half (58.1%) had anaemia, with transfusion requirement in 6.7% of patients. Admission to the intensive care unit was required in 46.9% of patients, with a mean length of stay of 2.96 days in intensive care unit and hospital stay of 9.04 days.

Conclusions

The use of prothrombin complex concentrate combined with common management measures, is effective and safe in the treatment of bleeding due to warfarin.
