It is for these same characteristics that unexpected findings such as thyroid incidentalomas are not infrequent, and their interpretation is a challenge.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,2</span></a></p><p id="par0010" class="elsevierStylePara elsevierViewall">A thyroid incidentaloma is defined as a focal or diffuse increase in the glandular uptake of FDG incidentally detected in a patient studied for a non thyroidal pathology.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1–3</span></a> In the PET/CT study the normal pattern of glandular uptake of FDG has a homogeneous distribution with an intensity slightly above the background activity.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4–7</span></a> This uptake is due to the use of fatty acids instead of glucose as the principal metabolic substrate.<a class="elsevierStyleCrossRefs" href="#bib0040"><span class="elsevierStyleSup">8,9</span></a></p><p id="par0015" class="elsevierStylePara elsevierViewall">Diffusely increased uptake of FDG is attributed to benign thyroid diseases such as Graves disease, thyroiditis and hypothyroidism, while focal uptake is more related to malignancy, representing a risk of cancer of 25–50% depending on the series.<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,10</span></a></p><p id="par0020" class="elsevierStylePara elsevierViewall">In this retrospective, descriptive study we present our experience in the identification, evaluation and follow up of thyroid incidentalomas detected in PET/CT studies with FDG.</p></span><span id="sec0010" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0070">Material and methods</span><p id="par0025" class="elsevierStylePara elsevierViewall">We retrospectively analyzed the PET/CT studies with <span class="elsevierStyleSup">18</span>F-FDG acquired in our department during the period from June 2010 to March 2013, selecting the cases with the presence of thyroid incidentalomas.</p><p id="par0030" class="elsevierStylePara elsevierViewall">The studies were acquired with a Gemini TF PET/CT equipped with a 64-slice CT. All the patients followed a standard protocol including: a 6-h fasting period, previous hydratation, glycemia <150<span class="elsevierStyleHsp" style=""></span>mg/dl at the time of tracer injection, resting in a supine position in silence during 50<span class="elsevierStyleHsp" style=""></span>min after the injection and bladder voiding prior to the study.</p><p id="par0035" class="elsevierStylePara elsevierViewall">A dose of <span class="elsevierStyleSup">18</span>F-FDG of 3.7<span class="elsevierStyleHsp" style=""></span>MBq/kg of weight was administered and low dose morphologic tomographic images were obtained (120<span class="elsevierStyleHsp" style=""></span>Kv/50<span class="elsevierStyleHsp" style=""></span>μAs) as well as PET at a speed of 2<span class="elsevierStyleHsp" style=""></span>min per bed from the base of the cranium to the proximal third of the thighs, with a total acquisition time of approximately 20<span class="elsevierStyleHsp" style=""></span>min.</p><p id="par0040" class="elsevierStylePara elsevierViewall">These images were processed with an iterative algorithm using the CT for correction of attenuation. Depending on the indication of the study and the organs of interest, this range of acquisition was broadened or delayed studies were performed.</p><p id="par0045" class="elsevierStylePara elsevierViewall">A whole body study was performed for the evaluation of thyroid incidentalomas, not taking into account the additional head and neck studies.</p><p id="par0050" class="elsevierStylePara elsevierViewall">Semiquantitative analysis of radiotracer uptake in the thryoid incidentalomas was done using the maximum standardized uptake value (SUVmax).</p><p id="par0055" class="elsevierStylePara elsevierViewall">Focal uptake was defined as the increase in glycidic activity above the physiologic background activity in a region of the thyroid bed with the extension not exceeding one thyroid lobe, and diffuse uptake was considered as a homogeneous increase above the background activity of the FDG uptake in both thyroid lobes (<a class="elsevierStyleCrossRef" href="#fig0005">Fig. 1</a>).</p><elsevierMultimedia ident="fig0005"></elsevierMultimedia><p id="par0060" class="elsevierStylePara elsevierViewall">A systematic search of the clinical history was made in all the cases registering the endocrinologic history related to the thyroid gland as well as the levels of thyrotropin (TSH) and antithyroid antibodies dated no more than 2 months before or after the PET/CT study.</p><p id="par0065" class="elsevierStylePara elsevierViewall">An ultrasonography was suggested in all patients with focal uptake as was fine needle aspiration puncture (FNAP) based on the ultrasound criteria.</p><p id="par0070" class="elsevierStylePara elsevierViewall">The statistical analyses were performed with the SPSS v21.0, with an initial descriptive analysis of the different variables and a posterior statistical relationship using the Student's <span class="elsevierStyleItalic">t</span>-test of the variables of interest.</p></span><span id="sec0015" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0075">Results</span><p id="par0075" class="elsevierStylePara elsevierViewall">During the period from June 2010 to March 2013 4.085 PET/CT studies were performed for reasons not related to the thyroid gland. Eighty-three patients (2.03%) were identified as having an increase in glycidic activity of the thyroid gland. This group presented a mean age of 64 years (range: 26–85), and 59 were women and 24 men. The patients with more than one study were counted only once.</p><p id="par0080" class="elsevierStylePara elsevierViewall">The indications for which the PET/CT studies with thyroid incidentalomas were requested are described in <a class="elsevierStyleCrossRef" href="#tbl0005">Table 1</a>. <a class="elsevierStyleCrossRef" href="#tbl0010">Table 2</a> shows the reasons for the request, the most frequent being the staging of neoplastic disease followed by suspicion of recurrence.</p><elsevierMultimedia ident="tbl0005"></elsevierMultimedia><elsevierMultimedia ident="tbl0010"></elsevierMultimedia><p id="par0085" class="elsevierStylePara elsevierViewall">Forty-six of the 83 patients (55%) showed a pattern of focal uptake and 37 (45%) a diffuse pattern.</p><p id="par0090" class="elsevierStylePara elsevierViewall">The causes of diffuse FDG uptake in the thyroid gland were: 24 hypothyroidism, 2 multinodular goiter, 2 diffuse euthyroid goiter and one post-thyroidectomy follicular adenoma. This etiological relationship was established thanks to the endocrinologic history reported in the clinical records with no other type of follow up or assessment.</p><p id="par0095" class="elsevierStylePara elsevierViewall">Thyroid ultrasound was performed in 34 of the 46 patients with focal FDG uptake, with FNAP being carried out in 23. The cytological characteristics were benignancy in 15 patients, 3 were compatible with papillary thyroid carcinoma and 2 showed metastastic lesions (one squamous carcinoma of the oropharynx and one poor differentiated lung neoplasm). The results of the FNAP are described in <a class="elsevierStyleCrossRef" href="#tbl0015">Table 3</a>.</p><elsevierMultimedia ident="tbl0015"></elsevierMultimedia><p id="par0100" class="elsevierStylePara elsevierViewall">The overall risk of malignancy of the thyroid incidentalomas was 6%, being 11% for the patients with focal FDG uptake.</p><p id="par0105" class="elsevierStylePara elsevierViewall">The mean SUVmax was 6.27 (range: 2.2–24), being 5.92 (range: 2.2–18.6) in the benign lesions and 10.26 (range: 3.3–24) in the malignant lesions. This difference was not statistically significant (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.311) (<a class="elsevierStyleCrossRef" href="#fig0010">Fig. 2</a>).</p><elsevierMultimedia ident="fig0010"></elsevierMultimedia><p id="par0110" class="elsevierStylePara elsevierViewall">Neither were statistically significant differences found between the SUVmax of the benign lesions with focal uptake and those with diffuse FDG uptake (<span class="elsevierStyleItalic">p</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>0.112).</p><p id="par0115" class="elsevierStylePara elsevierViewall">TSH serum levels (interval ≤2 months) were obtained in 29 of the patients with diffuse FDG uptake, with a mean value of 5.5<span class="elsevierStyleHsp" style=""></span>mUI/L (range: 0.5–31.36). No significant linear correlation was found between these levels and the SUVmax (px, <span class="elsevierStyleItalic">y</span><span class="elsevierStyleHsp" style=""></span>=<span class="elsevierStyleHsp" style=""></span>−0.56).</p><p id="par0120" class="elsevierStylePara elsevierViewall">It is of note that although both the focal and diffuse FDG uptake groups presented patients with a diagnosis of multinodular goiter, only those with focal uptake underwent ultrasonography and/or cytological follow up.</p><p id="par0125" class="elsevierStylePara elsevierViewall">No etiopathological relationship was found in 20 patients (24%), due to death in one and for causes related to the natural history of their neoplastic disease and/or comorbidity in the remaining patients. <a class="elsevierStyleCrossRef" href="#tbl0020">Table 4</a> shows the findings standardized by the pattern of metabolic activity, specifying the number and percentage of losses in each group.</p><elsevierMultimedia ident="tbl0020"></elsevierMultimedia></span><span id="sec0020" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0080">Discussion</span><p id="par0130" class="elsevierStylePara elsevierViewall">In the last years PET/CT with FDG has become a fundamental tool in the diagnosis and follow up of multiple diseases, especially neoplastic pathologies. The gradual increase in the use of this study has led to more frequent diagnostic challenges such as in thyroid incidentalomas.</p><p id="par0135" class="elsevierStylePara elsevierViewall">Physiopathologically, a diffuse increase in FDG uptake by the thyroid gland has been related to Graves disease with an increase in blood flow, a rise in the metabolic rate by hyperfunction and inflammatory changes brought about by autoimmune antibodies.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,11</span></a></p><p id="par0140" class="elsevierStylePara elsevierViewall">Likewise, the increase in the uptake of FDG in patients with hypothyroidism was based on studies of chronic Hashimoto or autoimmune thyroiditis which shows an increase in the number of lymphocytes within the glands, although no adequate correlation has been found with the serum levels of the antiperoxidase antibodies.<a class="elsevierStyleCrossRefs" href="#bib0060"><span class="elsevierStyleSup">12,13</span></a> It has not been ruled out that cellular apoptosis and active fibrosis may also contribute to the increase in metabolic activity.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a></p><p id="par0145" class="elsevierStylePara elsevierViewall">In the literature, diffuse FDG uptake in the thyroid has always been related to benign thyroid disease.<a class="elsevierStyleCrossRefs" href="#bib0005"><span class="elsevierStyleSup">1,12,13</span></a> However, no follow up studies have evaluated the probability of malignancy in this type of uptake.</p><p id="par0150" class="elsevierStylePara elsevierViewall">The physiopathological mechanisms determining a pattern of focal uptake have not been clearly described in the literature.</p><p id="par0155" class="elsevierStylePara elsevierViewall">As an additional finding an increase has been reported in the muscular glycidic and diffuse thymic activity, hypothetically related to the overexpression of glucose transporters regulated by thyroid hormones.<a class="elsevierStyleCrossRefs" href="#bib0075"><span class="elsevierStyleSup">15,16</span></a></p><p id="par0160" class="elsevierStylePara elsevierViewall">One of the main points to determine is the risk of malignancy of these findings. In the present study we found a global risk of malignancy of 6% which notably increased (11%) if only the focal uptake of FDG was taken into account. Although it is true that our values are lower than those reported by other series,<a class="elsevierStyleCrossRefs" href="#bib0020"><span class="elsevierStyleSup">4,10,17</span></a> the number of patients in our series was also lower. Nonetheless, we consider that this continues to be an important percentage, suggesting that thyroid ultrasonography and FNAP should be performed according to the ultrasound criteria in the group with a pattern of focal FDG uptake.</p><p id="par0165" class="elsevierStylePara elsevierViewall">In a systematic review of 22 studies carried out by Kerstin et al.,<a class="elsevierStyleCrossRef" href="#bib0010"><span class="elsevierStyleSup">2</span></a> the risk of malignancy was found to be 29.3% (range: 10.0–63.6%). The great width of this range was due to the difference in the number of patients and the problems of histopathological confirmation among the different series studied. In addition, in the last years, the indications for PET/CT have varied, and this change may also influence the incidence of thyroid incidentalomas.</p><p id="par0170" class="elsevierStylePara elsevierViewall">In the present study the SUVmax was not useful to discriminate between malignant and benign lesions. This may be explained by the reduced number of malignant lesions, wide ranges and extreme values in the two groups. To the contrary, in studies such as those by Choi et al.<a class="elsevierStyleCrossRef" href="#bib0050"><span class="elsevierStyleSup">10</span></a> and Kang et al.<a class="elsevierStyleCrossRef" href="#bib0090"><span class="elsevierStyleSup">18</span></a> a significant difference was observed between the SUVmax of these lesions. However, many other series also support our results.<a class="elsevierStyleCrossRefs" href="#bib0090"><span class="elsevierStyleSup">18–22</span></a> All the series present differences in the acquisition technique of the studies and the dose of FDG administered, thereby making correlations among the different series difficult.</p><p id="par0175" class="elsevierStylePara elsevierViewall">Diffuse benign uptake of FDG was more often related to hypothyroidism, with focal uptakes associated with multinodular goiter. This has been described by Karantanis et al.<a class="elsevierStyleCrossRef" href="#bib0070"><span class="elsevierStyleSup">14</span></a> who found that almost half of the patients with diffuse FDG uptake were under therapy with thyroxin for hypothyroidism, with these data being supported by the studies by Yasuda et al.<a class="elsevierStyleCrossRef" href="#bib0065"><span class="elsevierStyleSup">13</span></a> and Kim et al.<a class="elsevierStyleCrossRef" href="#bib0115"><span class="elsevierStyleSup">23</span></a>. However, the physiopathology of this uptake remains to be elucidated since although these authors attributed this uptake to hormone replacement therapies, the presence of antithyroid antibodies and the elevation of the T<span class="elsevierStyleInf">4</span> and T<span class="elsevierStyleInf">3</span> levels, these suppositions have yet to be confirmed.</p><p id="par0180" class="elsevierStylePara elsevierViewall">Deichen et al.<a class="elsevierStyleCrossRef" href="#bib0055"><span class="elsevierStyleSup">11</span></a> demonstrated <span class="elsevierStyleItalic">in vitro</span> that the TSH stimulates the uptake of FDG by the thyroid cells, supporting the clinical evidence that PET/CT is more precise than high TSH levels in well differentiated carcinomas which are still TSH sensitive. Despite these premises, in our study no statistically significant relationship was found between the grade of FDG uptake by the thyroid gland and TSH levels, which could also possibly be related to the low number of patients.<a class="elsevierStyleCrossRefs" href="#bib0065"><span class="elsevierStyleSup">13,14</span></a></p><p id="par0185" class="elsevierStylePara elsevierViewall">As anecdotic data, 2 patients in our series with diffuse FDG uptake presented hypothyroidism induced by sunitinib, a thyrosin-kinase inhibitor used in the treatment of gastrointestinal stromal tumors or renal cell cancer According to Desai et al.<a class="elsevierStyleCrossRef" href="#bib0120"><span class="elsevierStyleSup">24</span></a> and Faris et al.<a class="elsevierStyleCrossRef" href="#bib0125"><span class="elsevierStyleSup">25</span></a> this drug produces toxic thyroiditis presenting as subacute thyroiditis.</p></span><span id="sec0025" class="elsevierStyleSection elsevierViewall"><span class="elsevierStyleSectionTitle" id="sect0085">Conclusion</span><p id="par0190" class="elsevierStylePara elsevierViewall">The results of the present study demonstrate that thyroid incidentalomas, which are characterized by a focal increase in glycidic metabolism shown by PET/CT with FDG, present a considerable risk of malignancy (11%). This fact is independent of the SUVmax. The aim of this study was to show our experience in the detection of thyroid incidentalomas by PET/CT studies as well as its follow up.</p> <span class="elsevierStyleSectionTitle" id="sect0015">Material and methods</span><p id="spar0010" class="elsevierStyleSimplePara elsevierViewall">A retrospective and descriptive review was conducted on patients who had thyroid incidentalomas detected in <span class="elsevierStyleSup">18</span>F-FDG PET/CT studies between June 2010 and March 2013. Patient's medical records were reviewed for age, genre, maximum standardized uptake value (SUVmax), thyroid diseases, TSH and antithyroid antibodies levels, ultrasound, fine-needle aspiration (FNA) and cytology.</p> <span class="elsevierStyleSectionTitle" id="sect0020">Results</span><p id="spar0015" class="elsevierStyleSimplePara elsevierViewall">4085 PET/CT studies for several purposes were performed. Eighty-three of these studies (2.03%) showed thyroid incidentalomas. Thirty-seven patients showed a diffuse increase of glucose metabolism in the thyroid gland and 46 showed a focal increase of glucose metabolism. Five out of 46 patients with focal uptake were diagnosed of a neoplastic disease by cytology (11%). The SUVmax of malignant pathology did not differ from that of benign thyroid diseases (Mean: 10,26 and 5,92 respectively).</p> <span class="elsevierStyleSectionTitle" id="sect0025">Conclusion</span><p id="spar0020" class="elsevierStyleSimplePara elsevierViewall">In our experience, focal thyroid incidentalomas detected in <span class="elsevierStyleSup">18</span>F-FDG PET/CT studies are related to a significant risk of malignancy (11%).
