Objetivo

Presentación de un caso de osteólisis idiopática y de las posibilidades etiológicas y terapéuticas del síndrome de Gorham-Stout.

Caso clínico

Paciente de 74 años con una osteólisis progresiva de la cabeza femoral derecha sin ningún dato clínico de interés salvo el dolor y el acortamiento de 3 cm. Se realiza la sustitución protésica y se inicia el tratamiento con difosfonatos. El estudio histopatológico reveló una proliferación fibroangioblástica y elevada actividad resortiva llegando al Diagnóstico de osteólisis idiopática de la cabeza femoral (Síndrome de Gorham-Stout).

Resultado

Tras año y medio el paciente está asintomático y no se observa ningún signo de progresión de la enfermedad.

Conclusión

La osteólisis idiopática es debida al aumento de la actividad osteoclástica, por lo que el tratamiento antirresortivo con difosfonatos podría prevenir su progresión.

Objective

A case of idiopathic osteolysis is reported and the etiologic and therapeutic possibilities of the Gorham- Stout syndrome are discussed.

Case report

A 74-year-old patient had progressive osteolysis of the right femoral head with no clinical finding of interest other than pain and shortening by 3 cm. The patient was treated by hip replacement and biphosphonates. Microscopic study revealed fibroangioblastic proliferation and intense resorptive activity, resulting in the diagnosis of idiopathic osteolysis of the femoral head (Gorham-Stout syndrome).

Result

After a year and a half of follow-up, the patient is asymptomatic and there is no sign of disease progression.

Conclusion

Since idiopathic osteolysis is due to osteoclastic activity, antiresorptive treatment with biphosphonates may curtail its progression.
