Congenital anomalies of the vermiform appendix are extremely rare. Our patient is a 13-year-old male admitted for acute appendicitis. During appendectomy, we observed: a single base, appendix ‘outline’ with no inflammatory changes (4cm from its base), one end in the phlegmonous phase and the other end gangrenous (Fig. 1). The appendectomy was completed routinely. This anatomical variation corresponded with type A of the Cave–Wallbridge classification; however, as far as we know, an appendix outline associated with its duplication has not been documented, so ‘triplication’ could be contemplated as a possible subdivision of this category and should be distinguished from the ‘triple appendix’ variation recently described.
